GSG Consultants, Inc. (GSG) was retained by Jessie Brown Veteran’s Affairs Medical Center to conduct a limited asbestos survey at the Jessie Brown Veteran’s Affairs Medical Center located at 820 S.

Damen Ave. in Chicago, Illinois. The survey included testing for asbestos-containing materials (ACM) that may potentially be disturbed during renovation activities throughout the facility.

The survey was conducted by Mr. Eric Moore, who is an Illinois Department of Public Health (IDPH)

Licensed Inspector for Asbestos.

Asbestos-Containing Materials (ACM): The asbestos survey identified the presence of non-friable

ACM. In general materials that were sampled for ACM included: vinyl flooring & mastic. Refer to summary of findings in Section 3 Table 3.1 for materials sampled.

1.0 INTRODUCTION

GSG Consultants, Inc. (GSG) was retained by Jessie Brown Veteran’s Affairs Medical Center to conduct a limited asbestos survey at the Jessie Brown Veteran’s Affairs Medical Center located at 820 S.

Damen Ave. in Chicago, Illinois. The survey was conducted by GSG on July 7, 2016.

1.1 Objectives

The purpose of the asbestos survey was to identify the location of ACM that may require abatement prior to renovation activities. The scope of this survey was limited to the location of the sampling described herein. Sampling of building materials was performed in both the Damen and the Taylor

Buildings.

1.2 General Qualifications

The conclusions in this report are generally based on conditions observed within the facility. Test results contained within this report represent specific area(s) as identified by the assigned sample numbers. Any additional potential asbestos materials encountered that will be disturbed during renovation activities and that differs from materials sampled during this limited survey, will require further sampling and analysis. The estimated quantities provided herein should be considered approximate. This report has been prepared with generally accepted industry practices and procedures. No other warranty, either expressed or implied, is made.

The report is divided into five sections which discuss the survey activities and methodology, findings, and conclusions and recommendations associated with each of the subject areas addressed during this survey, as follows:

2.0 SURVEY METHODOLOGY

This section describes GSG’s asbestos survey approach and methodologies that were utilized during the field investigation activities. The asbestos survey included performing the following task:

ACM inspection throughout the Jessie Brown VAMC and it was limited to the flooring and mastic only.

Sample locations were selections by Mr. Kyle Russell, VAMC representative.

The following section present an overview of GSG’s approach for the type of survey completed as part of this project.

Asbestos-Containing Materials

The asbestos survey consisted of two phases: the first phase included a walk-through inspection to identify suspect ACM. The second phase consisted of collecting bulk samples from suspect ACM. The field survey was conducted by an IDPH-licensed asbestos inspectors. Sampling was conducted as per

EPA/NESHAP protocols. The samples were collected and stored in pre-cleaned containers with an assigned sample identification number prior to delivery to a laboratory. All sampling equipment was wet wiped and cleaned before and after each sampling event.

Since the scope of work included only the flooring, it is possible that some ACM were not discovered during the survey. Suspect ACM materials are likely to be concealed behind walls, above ceilings, or other inconspicuous locations throughout the building. Any additional suspect materials encountered that may be disturbed during renovation activities and that differs from materials listed in Table 3.1 will require further sampling and analysis.

The bulk samples were analyzed by both Polarized Light Microscopy (PLM) and Transmission

Electron Microscopy at an American Industrial Hygiene Association (AIHA) and National Voluntary

Laboratory Accreditation Program (NVLAP) accredited laboratory. Refer to Appendix E for laboratory licenses and accreditations. The samples were analyzed and reported by the layers present within each sample.

For the purpose of this survey, GSG derived its definition of ACM from the Environmental Protection

Agency (EPA) which classifies ACM as "any product containing more than one percent (1%) asbestos by volume, when analyzed by Polarized Light Microscopy (PLM) or Transmission Electron

Microscopy (TEM).”

Laboratory reports include, among other information, field sample numbers, and analysis results by layers, and the analyst’s signature.

3.0 SUMMARY OF FINDINGS

Asbestos-Containing Materials

Bulk samples of suspect ACM were collected and analyzed for the presence of asbestos. Materials that may be impacted during this proposed work were sampled following the Owner’s direction.

Results are summarized in Table 3.1 and include a description of each material, location, material type, and test results. Each suspect material was placed into one of three material categories: thermal systems insulation (TSI), surfacing materials (SURF), or miscellaneous materials (MISC). Materials confirmed to contain greater than one percent (1%) asbestos by PLM analysis and TEM analysis are indicated to have a “positive” result and are therefore classified as ACM. Materials indicated to have a “negative” result were confirmed by PLM analysis to be non-asbestos containing. Refer to Appendix

4.0 CONCLUSIONS AND RECOMMENDATIONS

Asbestos Containing Material

The asbestos survey identified the presence of non-friable ACM. The floor tile mastic tested positive for ACM. Refer to summary of findings in Section 3.0 Table 3.1 for materials sampled.

GSG recommends the preparation of an asbestos abatement project design prior to any renovation/demolition activities in which ACM may be impacted. Asbestos abatement design plan and specification shall include information regarding the location of containments and barriers, type of sealant, and air sampling requirements and clearance during the asbestos abatement activities.

The asbestos design plan and specification shall be prepared and signed by an IDPH licensed asbestos project designer in accordance with all applicable federal, state, and local regulations. Asbestos abatement work shall be conducted by a licensed abatement contractor under the supervision of a licensed asbestos project manager in accordance with all applicable Federal, state, and local regulations.

Any suspect material that is discovered during the renovation/demolition activities and is not listed in table 3.1 were not tested during this survey. Such materials shall be assumed and treated as ACM until tested and proven otherwise.

For any ACM that will remain in the building following renovation, GSG recommends development of an Operations & Maintenance Plan for the purpose of managing ACM in good condition and preventing future fiber releases.
