Statement of Need
STATEMENT OF NEED
A. GENERAL INFORMATION
1. PROJECT TITLE: James A. Haley Veterans’ Hospital, Facilities Management Service, 13000 Bruce B. Downs Blvd, Tampa, Florida 33612, has a requirement for over-the-door alarm systems patient rooms, shower rooms, and treatment rooms and a new CCTV Security Camera System at the Acute Recovery Center (ARC).
2. BACKGROUND: James A. Haley Veterans’ Hospital, Facilities Management Service has been directed to install Secure Emergency Door Alarms (SEDA) for patient safety and a new CCTV Security Camera System at the Acute Recovery Center (ARC).
3. PURPOSE: The equipment will enhance the ability to monitor patient safety while in a healthcare or behavioral care setting.
4. SCOPE: James A Haley is requesting the contractor shall provide all resources (labor, materials/parts, tools, equipment, and transportation) necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. The vendor shall provide over-the-door alarm hardware to equip the patient bedrooms, shower rooms, and treatment rooms in the Acute Recovery Center (ARC) to meet VA requirements in accordance with May 2019 memorandum and latest VA Mental Health Facilities Design Guide.
B. FUNCTIONAL AND PERFORMANCE CHARACTERISTICS:
1. Secure Emergency Door Alarms (SEDA)
a. When installed, the units will function to full manufacturer's specifications and meet all mental health safety requirements. The units provided shall be compatible with existing doors. At a minimum, system hardware shall also include strobe/indicator lights for each door, reset key switches for each door, nurse station consoles, all necessary power supplies, controllers, and any other hardware and software to provide a fully functional system after installation. All parts must meet or exceed manufacturer’s factory specifications to provide multiple door monitoring and with future performance/reliability upgrades.
b. There are thirty-six (36) doors that require sensors to prevent attempts to drape an item over the door. Each sensor shall be equipped with an adjustable nuisance delay to avoid false alarms. Sensors shall be pressure activated and provide full door coverage to include coverage over the hinge side of the door. The patient room doors are three-foot nine-inch (3’ 9”) doors; sixteen (16) have right hinges and twelve (12) have left hinges. The shower room doors are three-foot zero-inch (3’ 0”) doors; three (3) have right hinges and two (2) have left hinges. The treatment room doors are approximately three-foot zero-inch (3’ 0”) doors (one appears to be slightly smaller); two (2) have right hinges and one (1) has a left hinge. The exposed portion of the sensor shall be beige finish or similar color to coordinate with standard door finish.
c. Each door sensor will have an indicator light and reset key switch that is designed to be mounted near each covered door. Each key switch must be able to accept the standard VA core, IC (interchangeable) seven-pin (7-pin) small format core. Each indicator light will illuminate when the corresponding door sensor is activated.
d. There will be two (2) nurse station consoles provided that will have the capability to monitor all door sensors in real time. The console shall have both visual and audible displays of alarms as well as the ability to silence alarms. Alarm volume shall be adjustable. The system shall include alarm activation recording so that reports may be obtained.
e. Power supplies, controllers, and any other hardware required for a fully functional system shall be provided.
f. Hardware provided must be designed and tested for use in a mental health environment. Hardware components provided must be designed and tested to be a fully functional system meeting all the requirements in this statement of work. All hardware provided shall meet VA and industry standards to be tamper proof for use in an inpatient mental health facility.
g. Pressure sensors shall be tamper-proof or protected in such a way as to prevent access by patients. All exposed hardware surfaces in the patient area shall be anti-ligature.
h. Ligature resistant door alarm: Hardware/system components supplied must meet the following VA specifications:
1) Provide ligature resistant, monitoring and notification system capable of detecting pressure at the top of a patient bedroom door. An alarm system will trigger audio and visual notification devices at the bedroom door and nurse’s station to alert staff of a ligature emergency. The system shall be addressable, self-monitoring, and able to diagnose alarm and system problems. The system shall be capable of documenting ligature-initiated events.
2) Door Head Alarm Assembly:
a) Nominal 1 lb. pressure activated alarm.
b) Door head assembly must be constructed of stainless steel and made by the door alarm manufacturer. Assemblies made of plastic are not acceptable.
c) Optical sensors alarms are not acceptable.
d) The alarm assembly circuit shall be concealed and redundant providing a tamper resistant, failsafe operation.
e) The alarm and header assemblies are to be installed with tamper resistant fasteners only.
f) The header assembly shall eliminate ligature points available when the door is in the open position.
3) Hinge and Power Transfer
a) Power transfer section of hinge shall be housed at the top end of the continuous hinge to eliminate the potential of exposed wires or flex conduit.
b) The power transfer section of the hinge shall be field removable to eliminate the need to remove the door when addressing electrical service issues.
4) Local Visual Alarm (Strobe)
a) Install in corridor above monitored room doors throughout facility.
b) Alarm unit shall be anti-ligature with a sloped top, made of resilient material and fastened with tamper resistant hardware.
c) Door alarm units shall flash when any monitored door alarm is triggered at a ligature point initiated event.
d) The door alarm strobe shall be turned off when a user’s code is entered at the keypad at the room door.
5) Local Keyswitch:
a) Wall mounted, shall be flush mounted or designed to be anti-ligature.
b) Coordinate specific location with COR and Unit Manager
c) Designed for momentary actuation with spring return.
6) Keypad:
a) Provide an LCD display notification for activation of all monitored patient bedroom doors.
b) Flush mounted or designed to be anti-ligature
7) Remote Monitoring Panel and Audible Alarm:
a) The remote monitoring panel shall allow monitoring of each patient bedroom door from the nurse’s station with a visual indicator of the location of the ligature-initiated event.
b) The remote audible alarm will be a distinct tone not to be confused with other alarms located in the vicinity.
8) Control Panel:
a) Panel and all elements of the alarm system shall be equipped with a dedicated battery backup system and or emergency power feed for maintaining power to the control panel in the event of a power failure due to a power outage.
b) Provide software for printing documented alarm events
2. Camera Package System: The Contractor, at its expense shall furnish and install required equipment to the point of delivery specified in the contract. All cameras will be high quality network, power over ethernet (POE), with minimum of three-year (3-year) warranty. Cameras or camera components shall not be made in China. Cameras will be tamper-proof or able to be installed in a tamper-proof manner that is safe for the mental health environment. Products made in China is technically unacceptable and will be rejected.
a. One (1) each Axis Recorder Camera Station preloaded with licenses for 32 channels and necessary software (manufacturer part number 01613-001/S1132)
b. Eight (8) each Axis DN Dome Camera with Varifocal 3.5-10mm lens, with built in IR. IK10 impact-resistant casing, 8 MP at 30FPS with WDR (manufacturer part number 0888-001/P3228LVE)
c. Four (4) each Axis Fixed Dome Camera- 12 MP with WRD PTZ, Multi-stream 360/180DEG POE (manufacturer part number 01178-001/M3058-PLVE)
d. Four (4) each Axis 8 MP Multidirectional Network Camera with IR for 360-degree coverage, one IP Address, Varifocal 3-6mm lens, Forensic WDR, and Axis Lightfinder (manufacturer part number 01504-01/P3717-PLE)
e. Two (2) each Axis audio and I/O Interface with two-way audio streaming and configurable I/O ports (manufacturer part number 01160-001/T6101)
f. Two (2) each Axis Mk II Microphone 3.5mm with hemispherical directionality and dust and waterproof casing (manufacturer part number 01560-0001/T8351)
g. Two (2) each Axis POE+ (managed Gigabit) Network Switch with 24 ports, 2 RJ45/SFP24 combination ports, with POE+ (manufacturer part number 01192-004/T8524)
h. Eight (8) each Western Digital Purple Surveillance 8 TB Hard Drive with SATA 6 GB/S interface, Intel Power 64MB (manufacturer part number WD81PURZ)
i. Seven (7) each Axis Minidome Network Camera, with 3-6mm Varifocal lens, 3 MP/HDTV 1080P, OptimizedIR and WDR (manufacturer part number 01241-001/M4206-LV)
j. Two (2) each 40-inch TV Monitors
C. CHANGES TO STATEMENT OF WORK
Any changes to this SOW shall be authorized and approved only through written correspondence from the Contracting Officer (CO). A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor.
D. GOVERNMENT RESPONSIBILITIES
The government will verify that system hardware provided meets the required specification. The government shall provide appropriate electrical and any other utility service for the devices in accordance with the manufacturer’s requirements.
E. CONTRACTOR RESPONSIBILITIES
a. The Contractor’s personnel shall wear visible identification at all times while on the premises of the JAH VAMC. It is the responsibility of the contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police Service. The VA will not invalidate or make reimbursement for parking violations of the Contractor under any conditions. Smoking is prohibited inside any buildings of the VAMC. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violation of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, State or Municipal court.
F. PAYMENT
a. The Contractor will be paid 30 days in arrears upon receipt of a paper invoice.
b. Submit electronic invoices to OB10 – A third party provider contracted by Veterans Affairs (VA) to assist with electronic invoices, http://www.ob10.com/veteransaffairs NOTE: Invoices for payment may be rejected if all information required by FAR clause 52.212-4(g) is not included. Invoices shall include the following information to be considered valid for payment.
