STATEMENT OF WORK
Background: James A. Haley Veterans Hospital and Clinics maintains an annual service contract for the transport to – from hospital and associated clinics on business days.
Objective: This service contract is to ensure the timely, safe and reliable transport, while meeting all safety requirements, industry standards and regulations, and government requirements.
Scope of Work. The contractor will provide all supervision, labor, appropriately sized air conditioned vehicles, gasoline, insurance, handling materials, transport handcarts, ensure required training is completed and security requirements are met to provide same day courier service as follows:
1. One run daily – Monday through Friday - will require mid to large vehicle – Pick-up departing at 9 AM from James A. Haley Veterans’ Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL 33612, warehouse with 6 - 10 laboratory specimen Saf-T (insulated cases) traveling to VA Primary Care Annex 13515 Lake Terrace Lane, Tampa, FL 33637, Laboratory Assession, room 1B-606. You will drop off and pick-up the full (2 – 3) to be returned to James A. Haley Veterans’ Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL 33612, Laboratory room 1D-145 by 10 AM.
2. Once daily Supply route- Monday through Friday – will require a large vehicle –will be to-and-from, and will require delivery of laboratory specimen cases, locked bag interoffice mail, pharmacy medications, medical supplies from Logistics and Prosthetics, Clean/Dirty Linen, process store supplies - all items located in holding areas (see descriptions below). This run will depart at 10AM from James A. Haley Veterans’ Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL 33612 warehouse and proceed to Lakeland Community Based Outpatient Clinic (CBOC) 4237 South Pipkin, Lakeland, FL 33811 (drop-off/pick-up in 2 buildings) It will return making a stop at VA Primary Care Annex 13515 Lake Terrace Lane, Tampa, FL 33637, Laboratory Assession Area room 1B-606 to pick-up 2 – 3 Saf-T specimen containers at 1130 AM and return to James A. Haley Veterans’ Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL 33612, warehouse at approximately 12 noon with drop off in accordance with the requirements stated below.
3. Once daily Supply route - Monday through Friday - will require a large vehicle – will be to-and-from, and will require delivery of laboratory specimen cases, locked bags interoffice mail, pharmacy medications, medical supplies from Logistics and Prosthetics, Clean/Dirty Linen, process store supplies - all items located in holding areas (see descriptions below). This run will depart at 12:30 pm from James A. Haley Veterans’ Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL 33612, warehouse and proceed to the Brooksville CBOC located at 14540 Cortez Blvd, Suite 108, Brooksville, FL, 34613 and then proceed to the Zephyrhills CBOC 6937 Medical View Lane, Zephyrhills, FL 33542 and return to James A. Haley Veterans’ Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL 33612, warehouse at approximately 3:30 pm. (note Mailroom is open till 4 pm and warehouse is open till 4:30 pm) in accordance with the requirements stated below.
4. Once daily laboratory specimen run – Monday through Friday – with 330 PM pick-up from Lakeland CBOC 4237 South Pipkin, Lakeland, FL 33811 Laboratory room 152 of 1 – 2 Saf-T cases and then travel to VA Primary Care Annex 13515 Lake Terrace Lane, Tampa, FL 33637, Laboratory Assession Area room 1B-606 to pick-up 2 – 3 Saf-T specimen containers and depart at 430PM and deliver to James A. Haley Veterans’ Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL 33612, Laboratory room 1D-145. Courier will be allowed to park near the main entrance for delivery to Laboratory room 1D-145 by 5PM.
5. Once daily South Hillsborough (SOHI)CBOC run – Monday through Friday – with 9:30am, 12:00 noon and 4:30 pm pick from Summerfield Crossing Blvd, Riverview, Fl (LAB Rm 1B-606) to be deliver require delivery of laboratory specimen cases to James A. Haley Veterans’ Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL 33612, from (7-105) Courier will be allowed to park near the main entrance for delivery to Laboratory room 1D-145 by 5:30PM.
6. One Saturday laboratory specimen run with 1st pick-up stop at 12 noon at the Lakeland CBOC 4237 South Pipkin, Lakeland, FL 33811 and 2nd pick-up stop at 1 PM at VA Primary Care Annex 13515 Lake Terrace Lane, Tampa, FL 33637 Laboratory room 152 with drop off at James A. Haley Veterans’ Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL 33612 130PM. Courier will be allowed to park near the main entrance for delivery to Laboratory room 1D-145 by 2PM.
7. One Saturday South Hillsborough (SOHI)CBOC run from12920 Summerfield Crossing Blvd, Riverview, Fl (SOH 12;30 pm and area )with drop off require delivery of laboratory specimen cases at James A. Haley Veterans’ Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL 33612 drop off at 1:30 pm Courier will be allowed to park near the main entrance for delivery to Laboratory room 1D-145 by 2PM.
8. One run daily – Monday through Friday - will require mid to large vehicle – Pick-up departing at 8:00 AM from James A. Haley Veterans’ Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL 33612, warehouse with 6 - 10 laboratory specimen Saf-T (insulated cases) traveling to VA Lecanto Clinic, 2804 W. Marc Knighton Ct. Lecanto, FL. 34461. Laboratory Assession.
9. Two pick-ups daily from VA Lecanto Clinic, 2804 W. Marc Knighton Ct. Lecanto, FL. 34461.The first pickup must be at approximately 11am with delivery to the VA OPC Pasco, 9912 Little Rd New Port Richey, FL 34654. (Lab Rm A122) and James A. Haley Lab. The second pick up must be at approximately 3pm with delivery to the James A. Haley Lab. We will also need the courier to return any shipping containers to the Lecanto Lab each day. The days are Monday through Fridays, no weekends or holidays.
Description of items to be picked up and delivered as part of Supply runs - must be in air-conditioned vehicle- and vehicle must be large enough to transport ALL items in holding area.
1. Laboratory specimens cases will not exceed 2 1/2x2 1/2 x1 1/2, each weighing approximately 20 - 30 pounds. Delivery location and contact # are as follows:
a. James A Haley Warehouse pick-up empty cases on/near medal shelf marked for appropriate clinic for AM runs

2. Inter-office mail will be in a locked bag weighing from 5 – 20 pounds and is to be picked-up and dropped off as noted:

3. Intermittent delivery of pharmacy medications – medications will be in an enclosed container and will weigh approximately 5 pounds.

4. Medical supplies from Logistics will consist of 2 – 6 containers daily, weighing up to 25# and will be delivered to the outlying clinics and bagged equipment for repair and empty tubs will be returned:
a. James A Haley Warehouse pick-up full containers on medal shelf marked for appropriate clinic and equipment for repair and returned empty containers go to GC-003
b. Brooksville CBOC deliver to & pick-up empties in Lab room 110
c. Lakeland CBOC (bldg. 1) deliver to and pick-up empties in room 111 (Nursing or Lab staff will unlock door as needed)
d. Lakeland MSOC (Bldg 2) deliver to appropriate service – Podiatry room 157, Radiology, and HBPC room 138 (information noted on delivery bins) pick-up empty containers in room 136 (unlocked by radiology staff)
e. Zephyrhills CBOC deliver to & pick-up empties Lab room 105
5. Prosthetics supplies consist of wheelchairs, walkers and assorted other medical devices up to 20 pounds and may be in medium to large boxes or assembled and folded - there may be 2 -3 items as often as 2 – 3 times a week.

A clipboard will be located on each of the clinic carts located in the warehouse. VA staff is to sign-in what they are placing on the carts for delivery i.e. pharmacy container, 6 supply tubs, 1 wheelchair etc. Courier will ensure he is taking all items (listed/not listed that are on the cart) draw a line and sign and date/time. Next day’s list will begin immediately below this line and will continue to the next sheet if necessary. No sign-in/out sheets will be required at the clinic sites. Staff at these sites will report problems to the Contracting Officer’s Representative (COR). Courier service is expected to notify the COR at 813-972-2000 ext. 6209 of any delays or problems.
2. Work Hours: The service is to be provided daily, Monday thru Friday, excluding federal holidays or any other prearranged days, for which the VA has given the contractor 14 days’ notification that service will not be needed. The government will not be responsible for payment for federal holidays or any of the prearranged days. The federal holidays are New Year’s Day, Martin Luther King’s Birthday, Washington’s Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans’ Day, Thanksgiving, and Christmas, in addition to any other day specifically declared by the President of the United States to be a national holiday. The Saturday Route will be for all Saturdays except for 6 which are in conjunction with the following holidays; New Years, Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas. These dates will be confirmed with Courier Service.
3. Qualifications: The contractor must be established in the business called for, be financially responsible and have the necessary equipment and personnel to furnish the service required in this contract. The contractor shall meet all requirements of Federal, State and City code regarding operation of this type of business. The contractor shall be responsible for protecting the personnel furnishing the services under this contract. To carry out this responsibility, the Contractor shall provide the following for these personnel, as a minimum: workers compensation, income tax withholding, and social security payments. The parties agree that contractor personnel shall not be considered VA employees for any purpose and shall be considered employees of the Contractor. Contractor must possess at least the minimum amount of liability insurance for operation of courier services within the State of Florida (see VAAR Clause 852.237-71, Indemnification and Insurance, in accordance with VAAR Clause).
4. Payment: The Contractor will be paid each month, in arrears, upon receipt of a proper invoice. The invoice will be written to show the dollar amount for each trip at which supplies were picked up and delivered and the dates of the deliveries and pick-ups.
5. Training: The Contractor shall train courier employees in the safe transportation of bio-hazardous materials, including temperature control and notification procedures in case of accident or spills, in accordance with all applicable Local, State and Federal regulations. Contractor shall provide written documentation of each employee’s training in the safe transportation of bio-hazardous materials.
6. Confidentiality of Information: The Contractor shall ensure the confidentiality of all patient information being transported and will be held liable in the event of breach of confidentiality. The Contractor shall comply with the provision of the Federal Privacy Act of 1974 (Privacy Act, 5 U.S.C. 552a), HIPAA Privacy Rule (45 CFR Parts 160 and 164), as well as other statues regarding confidentiality of patient information during and after the execution of this contract. Because of the confidential medical record information that may be accessible and/or visible to contractor employees during performance of their daily duties, the contractor shall ensure all employees have completed the Government Training related to Privacy and compliance prior to allowing them to start work under this contract. Any information about an individual that is maintained by the VA, including, but not limited to, education, financial transactions, medical history, and criminal or employment history or any record that contains the person’s name, or the identifying number, symbol, or other identifying particular assigned to the individual, shall be kept CONFIDENTIAL. A copy of training certification shall be provided to the COTR upon request.
7. HIPAA Compliance: Health Insurance Portability and Accountability Act of 1996: The Contractor shall be required to be in compliance with HIPAA requirements and will be required to sign a Business Associate Agreement with the VA (attached) and return to the Contracting Officer. A signed copy of the Business Associate Agreement will be returned to the Contractor for their files.
8. Security Requirements for Contractor Employees: Contractor employees under this contract shall be the subject to fingerprint background investigations and must receive a favorable adjudication from the VA Office of Security and Law Enforcement prior to contract performance. This requirement is applicable to any subcontractor personnel, as well. If the investigation is not completed prior to the start date of the contract, the Contractor shall be responsible for the actions of the individuals they provide to perform work for the VA.
9. Failure to Provide Service: If the contractor fails to furnish the required services within the time frame indicated the government reserves the right to obtain the service from another source and to charge the contractor for any excess cost that may result. Furthermore, failure to perform any of the services set forth in this contract will be considered grounds for invoking provisions of default.
10. Drivers/Attendants: A record of each employee as to character and physical capabilities of performing the duties of a courier shall be maintained and made available for inspection upon request. All drivers shall have attained legal age (the age of 21 years) and shall maintain a neat, clean, and professional appearance. Drivers shall be licensed in accordance with the State of Florida, to include any special licensure required for transporting these items.
11. Employee Clothing and Identification: All contractor delivery personnel shall wear distinctive clothing bearing the contractor’s company name. All drivers shall be easily recognized as contractor employees by wearing appropriate identification which identifies them by name, title, and job performed.
12. Contractor Personnel: Contractor is completely responsible for all actions of their employees. All contractor personnel employed in the performance of this contract comply with all Veterans Administration (VA) building regulations, policies, and guidelines while on the premises. Contractor personnel shall be free of body odor and wear clean uniform while performing duties under this contract. Contractor shall consider the clinical environment and instruct/train their personnel in applicable safety precautions and special requirements. These requirements may include but are not limited to such conditions as human secretions, blood, barium, etc. Contractor personnel shall take appropriate precautions while performing duties applicable to this contract.
13. Insurance: The contractor shall procure and maintain for the entire contract period: Workmen’s Compensation, employer’s liability Insurance, general liability, and auto insurance. Prior to contract award, the contractor shall certify to the Contracting Officer in writing and provide documentation that the required insurance has been obtained. See VAAR 852.237-71, page 14.
