Statement of Work Replace Fiber Optic Cable (FOC) Install and terminate FOC from VA Police to Building #13 - # 14 At the West Palm Beach VA Medical Center in West Palm Beach, Florida

1.0 Overview

The VA’s building automation system at WPBMC has a failed fiber optic cable running from the main building to building 13/14 on site. Provide complete replacement and termination of the cable including any hardware and software to enable the system to communicate back to the main building automation server located in the energy center.

2.0 Parts, Labor & Material

a. This service shall cover all parts, components, and sub-components required in accordance with the manufacturer’s specifications.

b. All labor, travel, and per diem shall be included at No cost to the Government.

c. All equipment and special tools needed to perform, testing, inspections, cleaning etc... Shall be included at no cost to the Government.

d. All work areas will be kept broom clean and free of oil residue on machine and surrounding floors and surfaces, on a daily basis. All spills shall be cleaned immediately.

3.0 Special Instructions

a. All work shall be performed in strict accordance with all applicable codes and regulation with OSHA, ANSI, NEC, NFPA, ASTM, and ASME.

b. After hours work and weekends are at the discretion of the contractor and will not be cause for a change in contract. All overtime/weekend work shall be coordinated with the COR and is NOT a guarantee for approval.

c. The VA will provide no tools, equipment, or labor except as specifically noted within this statement of work.

d. Disruption to traffic flow when working in the parking structure shall be minimized and coordinated with COR.

4.0 Delivery

a. All work shall be coordinated with the COR. Normal Business working hours shall be Monday through Friday 8:30am to 4:00pm, or as bilaterally agreed by Siemens and VA.

b. Maintenance work on this contract shall only be completed between the months of January and June, contractor shall provide a schedule showing the adherence to this requirement.

5.0 Progress and compliance

a. The contractor shall report to the COR prior to the start of each days work. All work shall be monitored by the COR. A formal inspection and review by the COR shall satisfy the evaluation of completeness.

b. No additional services shall be made without the prior approval of the contracting office.

c. A VA issued ID badge is required for all contractors, service technicians and sub-contractor personnel. Two photo forms of identification are required.

d. All billing shall be in arrears. Upon receipt of contractor’s invoice, the COR will certify for payment, assuring that all services were performed in accordance with contract terms.
