ATTACHMENT A

Statement of Work Service Contract – Parking Garages

Objective:

Contractor shall furnish all labor, equipment, products, materials, and tools to clean all three Parking Garages at the Department of Veterans' Affairs Malcom Randall VA Medical Center, Gainesville, Florida, 32608. The Freedom Parking Garage is 219,207 square feet, the independence Parking Garage is 100,615 square feet, the Liberty Garage is 286,284 square feet.

Scope of Work:

· Provide and empty 60- gallon trash cans on each end of each floor daily Monday through Friday.

· Pick up trash on every level of every garage daily.

· Clean the bathroom in Freedom Garage daily Monday through Friday.

· Fill the toilet paper dispenser and paper towel dispenser as required Monday through Friday Contractor will provide paper products.

· Clean the stairwells every Monday and Friday.

· Clean the elevators and elevator lobbies (flooring, ceiling, and bright work) Monday through Friday.

· Remove all cobwebs weekly.

· Clean entrances and exits daily Monday through Friday.

· Dust all fixtures/light fixtures weekly.

· Clean the floors on each level every week (this must be done on weekends or after hours).

· Clean the bulkhead of each floor level as needed.

· Clean the ceilings as needed.

· Ensure Floor Drain Screens are free of debris.

· Any damage occurring to the parking garages due to contractor negligence, will be the responsibility of the contractor.

Safety Regulations

· The Contractor will provide all safety equipment as required by OSHA and VA regulations.

· All OSHA and VA safety regulations shall be observed during the performance of the service contract.

· All contractors must have completed the 10- hour OSHA safety course.

· The VA Safety Officer shall have full authority to see that the contractor obeys all safety rules and regulations relative to the fulfillment of this project.

· Provide the necessary barricades/signage where required.

Scheduling and Hours of Work

· All work in areas involved shall be scheduled with the Contracting Officers Representative.
