STATEMENT OF WORK FOR PATIENT LIFT SERVICE CONTRACT

INTRODUCTION-The Malcolm Randall and Lake City Veterans Medical Centers maintains and utilizes approximately 450 patient lifts throughout the medical centers including track lifts and floor based lifts. These lifts require semi-annual inspections, preventative maintenance and proper documentation to be provided on each individual patient lift.

BACKGROUND- The Malcolm Randall and Lake City Veterans Medical Center has a requirement for a semi-annual service contract to include inspections, preventative maintenance, weight testing, and proper documentation to be provided to the Medical Centers representative. The requirement or intent for this service contract is to provide a safe and secure environment for the Medical Center’s staff and patients. The Medical Centers is mandated by The Joint Commission to provide documentation of semi-annual visual inspections, annual preventative maintenance and annual load testing of each patient lift in use throughout the Medical Center. The Joint Commission requirement for documentation must state a “pass” or “fail” of each patient lift and any other historical data from testing. The documentation provided must be approved by the Medical Centers Representative. If the documentation provided is not satisfactory to the Medical Centers Representative than the awarding contractor will be required to reproduce the documentation with no additional charge to the government.

SCOPE OF WORK – The awarding contractor shall perform the following:

1. General notes and expectations:

a. Include parts and labor for Semi-annual safety inspections and Annual preventative maintenance and annual load testing.

b. During each maintenance visit the contractor shall clean, lubricate, adjust, repair and replace all parts as necessary to keep the equipment in first class condition and proper working order.

c. Service and emergency personnel shall report to the Electrical Supervisor or COR (Contracting Officers Representative) upon arrival at the Medical Center and again after completion of the required work. A copy of the work ticket containing a complete description of the work performed shall be given to the Electrical Supervisor or COR.

d. All work shall be completed by a qualified technician and a written report of all findings for each device or system shall be furnished to the Electrical Supervisor and Chief Engineer.

e. The testing described above shall be performed in a manner to eliminate disturbances to the medical center and its patients. Therefore, some testing may be required to be performed after normal working hours or on a weekend.

2. Semi-Annual Visual Inspection of the Track Lifts shall include the following:

a. All inspections and repairs shall be in accordance with the service contract and manufacturers recommendations/specifications.

b. Visually inspect the lifting tape for fraying or breaking along its entire length and replace as required.

c. Visually inspect the stitching of the lifting tape where it connects to the carrying bar for fraying or breaking and replace as required.

d. Visually inspect the sling for signs of wear and tear. The straps of the sling that connect to the carry bar of the lift shall be inspected for fraying or breaking and replace as required. Refer to manufactures recommendation/specifications on inspecting and replacing as required.

e. Visually inspect the hand control for damage and inspect the entire length of the cord for fraying, cuts or tears and replace as required.

f. Function test the hand control for proper operation (e.g. UP/DOWN/LEFT/RIGHT, etc.) and repair/replace as required.

g. Ensure the lifting system has no unusual sounds when in operation and repair/replace as required.

h. Visually inspect all structural brackets and support members of the lifting system to ensure the mechanical and structural integrity of the lifting system. Tighten or replace any faulty or loose component of the lifting system to ensure the mechanical and structural integrity of the lifting system.

i. Visually inspect the track system and charging system of the lifting system and repair/replace and faulty contacts or components of the lifting systems charging circuit.

j. Visually inspect the lifting system for cuts, dents or sharp edges on the carry bar that may damage the straps of the sling.

k. Visually inspect to ensure the end stops are in place and tightened. Install end stops or replace as required.

l. Visually inspect the lifting system to ensure it moves freely and smoothly the entire length of the track and make any adjustments required or lubrication that may be required.

m. Complete an inspection form on each patient lift recording all findings and required maintenance performed. Each inspection form shall list a “pass/fail” rating and shall be typed and be legible.

3. Annual Preventative Maintenance of the Track Lifts shall include the following:

a. All Preventative Maintenance and repairs shall be in accordance with the service contract and the manufacturer’s recommendations/specifications.

b. Inspect the entire lifting system operation per manufactures recommendations/specifications and operating manual.

c. Record and reset the PM lifts counter.

d. Check and load test the batteries and replace as required.

e. Lubricate all moving parts as required.

f. Wipe clean the entire track on the lifting system.

g. Check operation of the track limit switches.

h. Complete an inspection form on each patient lift recording all findings and required maintenance performed. Each inspection form shall list a “pass/fail” rating and shall be typed and be legible.

4. Annual Load Testing of the Track Lifts shall include the following:

a. All load testing shall be in accordance with the service contract, the manufacturer’s recommendations/specifications and EN10535 Standards.

b. Load test all tracks to 1.25 times the safe working load.

c. Each track attachment shall be point load tested.

d. The deflection of the track shall be measured, recorded and then determined if it is acceptable to EN10535 standards.

e. All beds utilizing a patient lift system shall not be out of service more than 2-continuous days.

f. Load test the track lift maximum load per the manufacturer’s specifications.

g. Inspect all above ceiling attachments and repair/replace as required.

h. Complete an inspection form on each patient lift recording all findings and required maintenance performed. Each inspection form shall list a “pass/fail” rating and shall be typed and be legible.

5. Semi-Annual Visual Inspection of the floor based Lifts shall include the following:

a. All inspections and repairs shall be in accordance with the service contract and manufacturers recommendations/specifications.

b. Visually inspect the sling for signs of wear and tear. The straps of the sling that connect to the carry bar of the lift shall be inspected for fraying or breaking and replace as required. Refer to manufactures recommendation/specifications on inspecting and replacing as required.

c. Visually inspect the hand control for damage and inspect the entire length of the cord for fraying, cuts or tears and replace as required.

d. Function test the hand control for proper operation (e.g. UP/DOWN/LEFT/RIGHT, etc.) and repair/replace as required.

e. Ensure the lifting system has no unusual sounds when in operation and repair/replace as required.

f. Visually inspect the lifting system for cuts, dents or sharp edges on the carry bar that may damage the straps of the sling.

g. Visually inspect the casters and ensure the lifting system rolls smoothly and effortlessly.

h. Visually inspect the structural integrity of mast, lift arm and base and repair/replace as required.

i. Visually inspect the actuator for structural integrity.

j. Visually inspect the actuator for wear or looseness at the lifting arm and repair/replace as required.

k. Visually inspect the lifting system to ensure all covers are installed and there are no “pinch points” within the lifting system. Repair/Replace as required.

l. Complete an inspection form on each patient lift recording all findings and required maintenance performed. Each inspection form shall list a “pass/fail” rating and shall be typed and be legible.

6. Annual Preventative Maintenance of the Floor based Lifts shall include the following:

a. All inspections and repairs shall be in accordance with the service contract and manufacturers recommendations/specifications.

b. Check and load test the batteries and replace as required.

c. Lubricate all moving parts as required.

d. Ensure the lifting system has no unusual sounds when in operation and repair/replace as required.

e. Visually inspect the casters and ensure the lifting system rolls smoothly and effortlessly.

f. Visually inspect the structural integrity of mast, lift arm and base and repair/replace as required.

g. Visually inspect the actuator for structural integrity.

h. Visually inspect the actuator for wear or looseness at the lifting arm and repair/replace as required.

i. Complete an inspection form on each patient lift recording all findings and required maintenance performed. Each inspection form shall list a “pass/fail” rating and shall be typed and be legible.

7. Annual Load Testing of the Floor based Lifts shall include the following:

a. All load testing shall be in accordance with the service contract, the manufacturer’s recommendations/specifications and EN10535 Standards.

b. Complete an inspection form on each patient lift recording all findings and required maintenance performed. Each inspection form shall list a “pass/fail” rating and shall be typed and be legible.

Safety Regulations All applicable safety regulations shall be observed during all work of this contract. The VA safety officer shall have full authority to see that the contractor obeys all safety rules and regulations relative to the fulfillment of this contract. Contractor will provide the necessary barricades/signage where required.

Scheduling and hours of work All work in areas involved shall be scheduled with the VA Electrical Supervisor. Work shall be scheduled for proper execution to completion of this contract.

The majority of work and maintenance can be performed during normal business hours. Although the contractor shall perform varying necessary repairs work after hours and / or during the weekends at the discretion of the VA Electrical Supervisor to eliminate disruption to the Medical Center.
