STATEMENT OF WORK

PORJECT: Specialty clinic VAV replacement

The purpose of this project is to remove three existing VAV terminal units and provide three new VAV terminal units with hot water reheat and DDC controls at the Specialty clinic on first floor of the RHJ VA Medical

Center.

SCOPE OF WORK:

1. Project includes demolition of three existing VAV terminal units entirely and provide three new VAV terminal units. Work will be done at the specialty clinic located on the first floor of the RHJ VA Medical center in Charleston, SC. Refer to attached schedule and drawing for complete scope of work.

2. Ralph H. Johnson Veterans Administration Medical Center retains the right to have all components examined by qualified technicians upon completion of work.

3. Contractor shall meet all Federal and State law for industrial safety and occupational health.

4. All work must conform to the VA design guides and specifications.

5. The components of the entire project installed under this contract shall be guaranteed for a period of one year including parts and labor from the date of acceptance by the Government.

6. Contractor shall submit equipment shop drawings for VAV boxes and DDC controls within ten calendar days after the project notice to proceed.

7. Electrical power for the new VAV boxes shall be extended from the electrical panel 1L11 sections 1/2 in electrical room D105B located in the Specialty clinic. A disconnect switch shall be provided for all individual VAV boxes. Disconnect switches must be mounted on the VAV boxes. All electrical work must be performed by a licensed electrical contractor.

8. All DDC work must be performed by Control Management, Inc. (CMI). Contact Ed Reamer at

(803)765-9070.

9. All VAV boxes shall remain operational during the normal working hours.

10. Background drawing is provided for information only. Notes applicable to this project are located in the text box on the upper righthand corner of the drawing.

11. Contractor parking and laydown area are not available on site.

12. If more than two ceiling tiles are taken out during the work of this project, plastic barrier shall be provided to isolate the work area. Plastic barrier material and installation shall be approved by the COR.

13. Thermostats for VAVs 1,2 and 3 shall be located in rooms D117, D123 and D127 respectively.

14. 30 hour OSHA card is required for the project superintendent and 10 hour OSHA card is required for all other contractors working on this project.

GENERAL:

1. Specialty clinic normal working hours are M-F 7:00AM-5:00PM. All work under this contract shall take place after normal business hours, M-F 5:00PM-10:00PM and weekends as directed by the COR. Actual after hour days/hours shall be determined by the COR depending on the Specialty clinic’s special requirements. Request to begin work must be submitted in writing to the Contracting Officer’s

Representative (COR) for approval.

2. All work shall be carried out to the satisfaction and approval of the Ralph H. Johnson VA Medical

Center COR. The contractor shall be responsible for demonstrating that the completed project meets the requirements as specified in the SOW and design documents.

3. Completion of this project shall not exceed 90(ninety) calendar days after notice to proceed.

4. A request for time extension must be submitted to the Contracting Officer in writing if required by the contractor. All requests must include specific and detailed reasons in support of the schedule alteration.

5. All noise producing work including drilling, cutting, hammering, etc. shall be coordinated and approved by the COR.

6. Contractor shall submit project schedule within ten calendar days after the project notice to proceed.
