timothy.russell@va.gov timothy.russell@va.gov This is a request for quote using Simplified Acquisition Procedures to procure Commercial Services for electrical distribution system testing/inspection services for the Huntington, WV. VA Medical Center.
Description of Work—
1. Vendor will provide all labor, materials, equipment and supervision to complete project.
2. Contractor shall provide all material, equipment, labor and technical supervision to perform specified tests and inspection for all work as required by drawings and specifications and as detailed per items listed below, includes but not limited to testing and certifying high voltage electrical components at VAMC Huntington WV.
3. Work includes the testing, inspecting, infrared scanning, adjusting, & repairing of High Voltage Transformers, Switchgear Equipment, Circuit Breakers, Protective Relays, Lightning Arrestors, & Transfer Switches. All infrared activity shall be accomplished in July, during peak heating load demand.
4. All electrical equipment, switchgear, primary switches, breaker, relays, connections, and such other devices as specified herein shall be cleaned, tested and adjusted so that the electrical distribution system is highly reliable and operating within industry and manufacturer’s tolerances.
5. Replace maximum temperature indicator on Transformer #1.
6. Replace C-phase arrestor on Transformer #3.
7. Replace B-phase surge arrestor on Transformer #5.
Technical Requirements—
A. Contractors shall be certified by the Inter-National Electrical Testing Association (NETA) as NETA Certified Technician and completed the Occupational Safety & Health Administration (OSHA) approved 10-hour construction safety training.
B. Contractors shall have technical trainings, and track records of working experience in maintenance, inspection, and testing of the Electrical Power Distribution System and its components in healthcare, industrial, educational, and commercial facilities for a minimum of five (5) continuous years.
C. Contractors shall have prior VA approved safety training – either on-the-job or class-room type - in electrical safety outlined in the OSHA Standard 29 Code of Federal Regulations (CFR) 1910 Subpart S – Electrical, and the NFPA 70E – Standard for Electrical Safety in the Workplace. Training certifications shall be submitted to the VA Contracting Officer prior to work. If no training certifications are available, the contractor’ Employer shall certify that he/she has met this requirement in writing and submit it to the VA Contracting Officer prior to work.
D. Contractor shall have ready access to the latest versions of the following references:
1. NFPA 70, National Electrical Code.
2. NFPA 70B, Recommended Practice for Electrical Equipment Maintenance.
3. NFPA 70E, Standard for Electrical Safety for the Workplace.
4. NFPA 110, Standard for Emergency and Standby Power System.
5. OSHA Standard 29 CFR 1910, Subparts I & S.
6. Inter-National Electrical Testing Association, Inc. (NETA) – Maintenance and Testing Specifications.
7. Operating /Maintenance manuals and specifications of the electrical equipment to be maintained and tested. These documents may be obtained from the VHA Medical Center, or the equipment manufacturers.
E. Contractors shall be equipped with all necessary tools, equipment, and Personal Protective Equipment (PPE) to perform the work safely, effectively, and timely. Tools, equipment, and PPE shall comply with the requirements of OSHA Standard 29 CFR 1910, Subpart I, and NFPA 70E.

B.2 STATEMENT OF WORK
PART A – GENERAL INFORMATION
A.1 INTRODUCTION – This Statement of Work includes all labor, materials, equipment, and qualified supervision for testing, inspecting, and required reports for a “3 Year Electrical Inspection” at the Hershel “Woody” Williams VA Medical Center (Huntington, WV) located at 1540 Spring Valley Drive, Huntington WV, 25704.
A.2 BACKGROUND – The VAMC has a need to have this work performed in order to meet the Joint Commission and NFPA Standards.
A.3 SCOPE OF WORK – Vendor will provide all parts, labor, equipment, material, and supervision to perform the work and make repairs as indicated below.
PART B – WORK REQUIREMENTS

1.2 PERFORMANCE
The Contractor shall verify conditions as they exist prior to bidding the project.
The Contractor shall provide all material, equipment, labor and technical supervision to perform specified tests and inspections.
All work performed shall be fully documented by the Contractor. Testing shall be performed in compliance with manufacturers’ recommendations and applicable NEMA, NEC, ANSI, ASTM, IEEE, NETA, OSHA, and VA codes & standards.
Test Report:
The test report shall include the following:
Company’s Name, address, telephone and fax number Name and signature of contractor who perform the maintenance and testing VA work contract number, name and number of VA contracting Officer.
Date and time of work.
Copies of contractor’s valid licenses, professional and training certificates.
Summary of project Descriptions and model number of specialized tools and equipment used, such as torque wrench or infra-red scanning camera Description of work items Test results Reference materials such as equipment manufacturer’s specifications, coordination study, etc.
Remarks on conditions of electrical equipment. List all deficiencies, if any.
Recommended corrective actions, if any.
Furnish four (4) copies of the completed bound report to VA no later than 15 days after testing is completed. Final acceptance will be withheld until the reports are submitted and approved by VA.
Upon completion of the tests and inspections noted in these specifications, a label shall be attached to all serviced devices. These labels will indicate date serviced and the Testing Contractor responsible.
The tests and inspections shall determine suitability for continued reliable operation.
All outages shall be scheduled and approved in writing by VA at least one week in advance. Outages will be scheduled only at night and on weekends.
SUBMITTALS
In accordance with Section 01 33 23, SAMPLES AND SHOP DRAWINGS, furnish the following certifications: Two weeks prior to final inspection, deliver four copies of the following certifications to VA of certification by the manufacturer that the equipment conforms to the requirements of the specifications. Provide complete copies of all test performed showing the required data as indicated in Part 3, Execution, including technical data sheets, appraisals and recommendations.

All testing equipment shall be UL listed and used in accordance with the National Electrical Code. All oil samples from transformers shall be sent to an independent laboratory with copies of tests provided to VA.

TRANSFORMER TESTING
The transformer numbers above correspond with the provided informational electrical schematic drawing “Primary Electrical one-line diagram 09”, dated July 2009.
All transformers listed above shall be inspected. Cables, connections, & switches shall also be inspected and cleaned.
Check all liquid in tanks and bushings for proper level and inspect for oil leaks.
Oil from transformers shall be sampled in accordance with ASTM procedures and analyzed by an independent laboratory for:
Color Acidity Interfacial tension Dielectric strength Dissolved gas-in-oil Power factor Test insulation resistance, power factor, and turns ratio.
Replace gaskets as needed.
Liquid filled transformers shall have the cooling liquid checked and replaced when test indicates that it no longer meets manufacturer’s requirements.
Dry type transformers shall be thoroughly cleaned. Make a close examination for the collection of dirt or other forms of material that may have collected in the windings during normal operation. Any foreign material that interferes with cooling or reduce clearance to ground should be removed using accepted methods.
Analyze oil for PCB contaminants and determine parts per million content in accordance with either ASTM or EPA procedures.
CABLE
Inspect exposed sections for physical damage.
Inspect for shield grounding, cable support, and termination.
Inspect for visual jacket and insulation condition.
Terminations shall be properly corona suppressed.
Perform shield continuity test by ohmmeter method. Record Ohm value.
There are approximately 18 sets of power cables to be tested.
SWITCHGEAR EQUIPMENT, CIRCUIT BREAKERS & PROTECTIVE RELAYS
Inspect for physical, electrical, and mechanical condition.
Inspect for proper alignment, anchorage, and grounding.
Clean all electrical connections.
Inspect all bus connections for high resistance, use resistance ohmmeter. An infrared survey shall be made to identify high resistance connections. Use torque wrench method to tight and correct any loose connection found. Provide an infrared photo for each individual piece of equipment.
All 4160, 480, and 120 Volt Breakers associated with each transformer and main switchgear shall be inspected, cleaned, lubricated, & repaired. All loose connection shall be tightened.
Inspect the insulation system on the primary bus and assemblies. Test insulation on each bus, phase-to-phase and phase-to-ground with suitable mega ohmmeter and record values and report & correct deficiencies.
Visually inspect the equipment ground and report & correct deficiencies.
(5 kV Breaker Only) Draw or rack each breaker form its cell. Remove arc chutes, clean, inspect, and adjust all contacts as necessary. Measure and record contact resistance in micro-ohms and clean all insulating surfaces. Measure and record phase-to-phase and phase-to-ground.
Electrically close and trip each breaker with control switch. Manually close and trip each breaker. Trip each breaker with each of its protective devices. Tighten all connections.
Inspect, clean and calibrate all Protective (Over current) Relays.
Perform Over-potential Test: High Pot all 4160v breakers. Test voltage shall be in accordance with manufacturer’s recommendations.
Check equipment for level, security to foundation, and operation of doors. Report and correct any unfavorable environmental conditions such as excessive moisture or conducting dust.
Main switchgear room at Building #26 consists of 18 cubicles of 5 kV switchgear.
Most switchgear equipment, circuit breakers, etc. to be tested are manufactured by Square D Company.
There are approximately 17 medium voltage switches, 7 medium voltage air circuit breakers, 6 medium voltage vacuum circuit breakers, 15 buses, 1 electronic trip circuit breaker, 20 low voltage circuit breakers and 55 protective relays.
EMERGENCY GENERATOR TRANSFER SWITCHES (22)
Inspect for physical, electrical, and mechanical condition.
Perform an infrared survey on connection while transfer switch is in both the normal and emergency position to identify high resistance connections and/or hot spots. Use torque wrench method to tighten and correct any loose connections found.
LIGHTNING ARRESTORS
Inspect for physical damage and clean.
Perform a Doble test Perform an insulation resistance test.
INFRA-RED SCANNING
This work item must be done while the Electrical Power Distribution System is energized. Appropriate safety precautions must be taken before, during and after scanning the system:
Use an infra-red scanning camera to detect hot spots in the Electrical Power Distribution System under normal loading conditions to indicate loose connections, insufficient contact pressures and excessive loading. Objective of this work is to detect any loose, broken, or corroded connections in the system. Problem connections shall be replaced with new connectors, and/or tightened with torque wrench to meet the equipment manufacturers’ specifications.
Provide photographs and engineering report to indicate problem areas for VA awareness.
PHASING
Coordinate disconnecting of main voltage source feeds from pole with American Electric Power.
Load adjustment will be arranged to compensate for the loss of power. This will be accomplished with tie-breakers and/or emergency diesel generators. Notify VA at least one week prior to switchgear testing to allow ample time for notifying the services affected and scheduling the operation of the generators.
Electrical testing and services will be performed in other than normal working hours of 8 a.m. to 4:30 p.m., Monday through Friday unless otherwise approved by VA.
APPENDIX #1
CIRCUIT BREAKER - MEDIUM VOLTAGE - VACUUM

PROTECTIVE RELAYS
VISUAL AND MECHANICAL INSPECTION
Relays shall be inspected for physical damage.
Inspect cover gasket, cover glass, and presence of foreign materials, moisture, and condition of spiral spring, disc clearance, contacts, and case shorting contacts if present.
Check mechanically for freedom of movement, proper travel and alignment, and tightness of mounting hardware and tap plugs.
Perform all settings in accordance with owner/user's provided settings.
B) ELECTRICAL TESTS
Perform the following tests on the nominal settings specified by owner/user's electrical engineer:
Pickup parameters on each operating element.
Timing test shall be performed at two (2) points on time dial curve.
Pickup targets and seal in units.
Special tests as required to check operation of restraint, direction, and other elements per manufacturer's instructions.

TRANSFORMERS - LIQUID FILLED
VISUAL AND MECHANICAL INSPECTION
Inspect for physical damage. Make a close examination for all damage that may have occurred as the result of environment, or other problems, which could interfere with normal operation.
Compare equipment nameplate information with latest single-line diagram and report discrepancies.
Verify proper manual operation such as fans, pumps, indicators, tap changer, and gas pressurization system.
Check all liquid in tanks and bushings for proper level and inspect for oil leaks. Verify that positive pressure is maintained on nitrogen-blanketed transformers.
Verify correct equipment grounding.
B) ELECTRICAL TESTS
Insulation resistance tests shall be performed winding-to-winding and winding-to-ground. Appropriate guard shall be utilized over all bushings. Test duration shall be for ten minutes with resistances tabulated at 30 seconds, one minute, and ten minutes. Calculate polarization index.
Insulation power factor tests shall be made on all high and low voltage winding and bushings. Overall dielectric loss and power factor (CH, CL, CHL) shall be determined. Ungrounded specimen tests (UST) shall be made on any bushing equipped with power factor or capacitance taps. Correct test results to 20 degrees C in accordance with test equipment manufacturer’s instructions.
A turn’s ratio test shall be performed between windings for all tap positions. Verify tap setting is as specified.
Insulating oil shall be sampled in accordance with ASTM D-923. Sample shall be laboratory tested for:
Dielectric breakdown voltage / Acid neutralization number Specific gravity / Interfacial tension / Color / Visual condition Perform dissolved gas analysis (DGA)/ PPM water

Perform specific inspections and mechanical tests as recommended by manufacturer.
Make a close examination for shipping brackets or fixtures that may not have been removed during original installation. Insure resilient mounts are free.
Make a close examination for the collection of dirt or other forms of material that may have collected in the windings during normal operation. Any foreign material, which may interfere with cooling or reduce clearances to ground, should be removed using acceptable methods.
B) ELECTRICAL TESTS
Insulation resistance tests shall be performed winding-to-winding and winding-to-ground.
Insulation power factor tests shall be performed winding-to-winding and from each winding-to-ground.
A turn’s ratio test shall be performed between windings.
Drawing “Primary Electrical one-line Diagram 09” dated July 2009, electrical schematic drawing can be provided as a reference indicating power distribution systems at VAMC, coordinate with COR.
Repair listed deficiencies noted as detailed within the specification 3.1.G.2. All work shall be accomplished in calendar year 2020.
Administration Progress and Compliance:
Vendor on-site person will have daily interaction with FMS staff to establish priorities and workload progress.
The vendor is also required to provide their own carts for infection control issues during work performed in public areas.
PART C – SUPPORTING INFORMATION
GENERAL CONDITIONS AND DEFINITIONS:
VAMHCS: Veterans Affairs Maryland Health Care System.
VA: Veterans Affairs CO: V.A. employee of the VA Contracting Service officially assigned to act as Contract Officer in the behalf Veterans Health Administration. The services performed are that of general supervision, direction, control and approval of the contract.
COR: Contracting Officers Representative. V.A. employee of VAMC Facilities and Engineering Service officially assigned to act as Project Manager and Inspector on behalf of VAMC and Contracting Officer.
Coordinate all administrative and construction requirements exclusively with or through assigned COR of VAMC Facilities and Engineering Service; and. neither discuss contract work with, nor take any directives concerning contract work from any other person than COR unless specifically permitted and authorized to do so.
PROVIDE: Whenever used throughout Scope of Work, term “Provide” is to be understood to mean -Purchase newly manufactured; Furnish delivered to VAMC work site in protective containers; Secure from tampering or theft; and Install in accordance with Contract Scope of Work, Drawings, Specifications, and Manufacturer’s Instructions all required items of administrative submittals, labor, and installation works for the Contract.
Any work the Contractor is required to accomplish due to unsatisfactory performance or nonperformance in accordance with the stated quality and level of effort specified in the contract. Rework is performed at no additional cost to the Government.
Follow all Infection Control Risk Assessment (ICRA) guidelines for dust control during all construction work. A construction dust “Risk Assessment” will be conducted by the Medical Center’s Infection Control Nurse. Temporary “dust” barriers may be installed based on this assessment. Bi-weekly inspections of barriers and work areas will be conducted by Infection Control Nurse, Safety Manager, and COR to enforce implementation of risk assessment.
Follow Interim Life Safety Measures. Any series of Operational Actions taken to temporarily reduce hazards posed by Life Safety deficiencies which have been created by demolition, renovation, or construction works, and which are to remain in place and in effect for duration of Contract work. Provide interim life safety measures (ILSM) where fire protection, fire suppression or smoke/fire partitions will be shut down or rendered ineffective during construction. Coordinate with VA safety office through VA COTR. ILSM must always be posted outside work area.
All equipment and materials shall be approved prior to being brought on job site.
Although normal work hours are Monday through Friday 8:00 a.m. to 4:30 p.m., for critical areas that will require shut-downs, work shall be completed on 2nd and 3rd shifts and weekends. Final schedule shall be determined after analysis of damaged drives. Contractor shall make prior arrangements with the COR to coordinate shut-down periods. VA will not grant access to subs, it is the responsibility of the contractor.
Contractor must obtain a burn permit from VA Safety Office prior to any welding, soldering work and burning.
All employees of general contractor and subcontractors shall comply with VA security management program and obtain permission of the VA police.
The work is in a professional environment, employees must conduct themselves accordingly. Employees shall not use abusive language, carry firearms, or bring alcoholic beverages on site. Employee's dress code shall consist of normal work uniform. Shirts must always be worn.
Contractor shall provide general clean up to construction area daily to the satisfaction of the VA Project Manager.
Hospital smoking policy (NO SMOKING IN THE BUILDING), smoking must be done in approved smoke shelters or at least 35 feet from any building.
OSHA and VA CONSTRUCTION SAFETY STANDARDS will be enforced. Contractor must have 30-hour OSHA Construction Safety course.
Start up, test, check and adjust all new units in the presence of the VA COR.
Place of Performance – The VAMC located at 1540 Spring Valley Drive, Huntington WV 25704.
Special Considerations – Contractor Furnished Materials – All labor, materials, equipment to perform the job. When “furnish”, “provide”, “install”, or similar term is used it shall mean a complete installation, ready for use. The vendor shall supply all parts, equipment, material and labor to conduct work as listed in the Statement of Work above. The work will include all items as described in the Scope of Work listed above.
Government Furnished Materials and Services. Government furnished property, if any, will be identified and will only be used for the performance of this contract unless otherwise authorized by the Contracting Officer. The Contractor shall, always, take any and all steps necessary for maintenance and preservation of all Government-furnished property. The Contractor shall comply with all reasonable requests of the Contracting Officer to enclose, or specifically protect, Government-furnished property.
Qualifications of Key Personnel – Contractor’s employees should be knowledgeable of standards associated with this type work.
Supervision/Communications. At all times during the performance, the Contractor’s Superintendent (if applicable) shall be available by cellular phone. Prior to beginning any work, the Contractor shall supply the COR with the telephone number for the Superintendent. The Contractor’s “Competent Person” that meets OSHA training guidelines, can make decisions and acting on behalf of the Contractor shall always be physically located on site during performance of the contract. If the Contractor’s Competent Person is absent from the job site for an extended period, the COR may send all Contractor/sub-contractor employees off the job.
Security Requirements: Contractor shall identify which employees will be working under the awarded contract. Contractor personnel shall include employees, sub-contractors, subcontractor employees, suppliers and delivery personnel entering the Medical Center. This does not apply to suppliers and delivery personnel making deliveries to the VA warehouse dock only. Those employees shall bring photo ID to obtain a VA Contractor identification badge that shall always be worn above the belt while on Medical Center property. Contractor’s employees shall return all I.D. badges to designated personnel at time designated in the task order.
The Contracting Officer’s Representative (COR) is responsible for the inspection of the work called for in this job. The Contracting Officer’s Representative will be identified in the task order by name, telephone number and location. Inspections made by the COR are for the sole benefit of the Government and do not relieve the Contractor of any quality control responsibilities.
The Safety Manger is responsible for safety inspections of all contract operations. The Safety Manager will be identified in the task order by name, phone number and location.
Drawings – General. Contract drawings (if applicable) will indicate the extent and location of work. If any departure from the contract drawings are deemed necessary by the Contractor, the details of such departure and the reasons therefore shall be submitted to the Contracting Officer for approval as soon as practical. No such departure shall be made without proper written permission of the Contracting Officer. The Contractor is responsible for all information and requirements in drawings and specifications that pertain to the task order.
Smoking Policy. The Contractor shall not allow smoking in any building. The Contractor shall ensure employees smoke only in designated outdoor areas, 50 feet away from buildings.
Parking/Traffic Regulations. The Contractor shall not park on grassy areas unless approved by the COR and the Contractor agrees to restore areas back to VA standards. Drivers should be particularly concerned with pedestrian traffic. Yield to pedestrians in crosswalks. Posted speed limits and all other traffic controls are always to be observed by operators. Seat belt use is mandatory on the VA grounds.

SECTION C - CONTRACT CLAUSES
C.1 52.212-4 CONTRACT TERMS AND CONDITIONS—COMMERCIAL ITEMS (OCT 2018)
(a) Inspection/Acceptance. The Contractor shall only tender for acceptance those items that conform to the requirements of this contract. The Government reserves the right to inspect or test any supplies or services that have been tendered for acceptance. The Government may require repair or replacement of nonconforming supplies or reperformance of nonconforming services at no increase in contract price. If repair/replacement or reperformance will not correct the defects or is not possible, the Government may seek an equitable price reduction or adequate consideration for acceptance of nonconforming supplies or services. The Government must exercise its post-acceptance rights—
(1) Within a reasonable time after the defect was discovered or should have been discovered; and
(2) Before any substantial change occurs in the condition of the item, unless the change is due to the defect in the item.
(b) Assignment. The Contractor or its assignee may assign its rights to receive payment due as a result of performance of this contract to a bank, trust company, or other financing institution, including any Federal lending agency in accordance with the Assignment of Claims Act (31 U.S.C. 3727). However, when a third party makes payment (e.g., use of the Governmentwide commercial purchase card), the Contractor may not assign its rights to receive payment under this contract.
(c) Changes. Changes in the terms and conditions of this contract may be made only by written agreement of the parties.
(d) Disputes. This contract is subject to 41 U.S.C. chapter 71, Contract Disputes. Failure of the parties to this contract to reach agreement on any request for equitable adjustment, claim, appeal or action arising under or relating to this contract shall be a dispute to be resolved in accordance with the clause at FAR 52.233-1, Disputes, which is incorporated herein by reference. The Contractor shall proceed diligently with performance of this contract, pending final resolution of any dispute arising under the contract.
(e) Definitions. The clause at FAR 52.202-1, Definitions, is incorporated herein by reference.
(f) Excusable delays. The Contractor shall be liable for default unless nonperformance is caused by an occurrence beyond the reasonable control of the Contractor and without its fault or negligence such as, acts of God or the public enemy, acts of the Government in either its sovereign or contractual capacity, fires, floods, epidemics, quarantine restrictions, strikes, unusually severe weather, and delays of common carriers. The Contractor shall notify the Contracting Officer in writing as soon as it is reasonably possible after the commencement of any excusable delay, setting forth the full particulars in connection therewith, shall remedy such occurrence with all reasonable dispatch, and shall promptly give written notice to the Contracting Officer of the cessation of such occurrence.
(g) Invoice.
(1) The Contractor shall submit an original invoice and three copies (or electronic invoice, if authorized) to the address designated in the contract to receive invoices. An invoice must include—
(i) Name and address of the Contractor;
(ii) Invoice date and number;
(iii) Contract number, line item number and, if applicable, the order number;
(iv) Description, quantity, unit of measure, unit price and extended price of the items delivered;
(v) Shipping number and date of shipment, including the bill of lading number and weight of shipment if shipped on Government bill of lading;
(vi) Terms of any discount for prompt payment offered;
(vii) Name and address of official to whom payment is to be sent;
(viii) Name, title, and phone number of person to notify in event of defective invoice; and
(ix) Taxpayer Identification Number (TIN). The Contractor shall include its TIN on the invoice only if required elsewhere in this contract.
(x) Electronic funds transfer (EFT) banking information.
(A) The Contractor shall include EFT banking information on the invoice only if required elsewhere in this contract.
(B) If EFT banking information is not required to be on the invoice, in order for the invoice to be a proper invoice, the Contractor shall have submitted correct EFT banking information in accordance with the applicable solicitation provision, contract clause (e.g., 52.232-33, Payment by Electronic Funds Transfer—System for Award Management, or 52.232-34, Payment by Electronic Funds Transfer—Other Than System for Award Management), or applicable agency procedures.
(C) EFT banking information is not required if the Government waived the requirement to pay by EFT.
(2) Invoices will be handled in accordance with the Prompt Payment Act (31 U.S.C. 3903) and Office of Management and Budget (OMB) prompt payment regulations at 5 CFR part 1315.
(h) Patent indemnity. The Contractor shall indemnify the Government and its officers, employees and agents against liability, including costs, for actual or alleged direct or contributory infringement of, or inducement to infringe, any United States or foreign patent, trademark or copyright, arising out of the performance of this contract, provided the Contractor is reasonably notified of such claims and proceedings.
(i) Payment.—
(1) Items accepted. Payment shall be made for items accepted by the Government that have been delivered to the delivery destinations set forth in this contract.
(2) Prompt payment. The Government will make payment in accordance with the Prompt Payment Act (31 U.S.C. 3903) and prompt payment regulations at 5 CFR part 1315.
