None required. You collected samples in 2016. You will be required to sample again in 2019

WATER QUALITY PARAMETERS

Biweekly pH readings shall continue to be collected after treatment and recorded on the

DOH 360 monthly report. Silica and orthophosphate concentrations must also be recorded biweekly if they are used.

MICROBIOLOGICAL

Number of samples-quarterly from each well, total 20 (4 x 5 wells) A quarterly sample must be collected after any GAC or air stripper.

7 per month from distribution

Analytes total coliform

A free chlorine residual determination must be made at the same time and location that any sample is collected for bacteriological analysis.

A daily free chlorine residual must be collected at each well field at the entry point to distribution.

Testing for Group 3 chemicals is optional for distribution samples.

NITRATE/NITRITE

Number of samples - Wells 1A and 2 need a single sample from each well.

Quarterly samples from Wells 3, 4 and 5.

Quarterly samples at the entry point to distribution.

Analytes nitrate/nitrite

Quarterly sampling is required from any well which exceeds 50% of the MCL for nitrates/nitrites. Annual sampling of wells that exceed 50% of the MCL is permitted if they remain in the voluntarily restricted category. One of the distribution samples should be collected during the summer.

DISINFECTION BYPRODUCTS

Your approved plan requires 1 sample collected in the first or second week of August at the following location: Northport VA Hospital, Building 65, Room 104 bathroom sink.

RADIOLOGICAL

1 sample collected from well #5. The NYSDOH will be conducting this analysis for you. As soon as we know the NYSDOH sampling schedule we will contact you to set up a day and time.

Well 3 was sampled in 2011. It will need to be sampled again in 2020.

Well 2 and 4 were sampled in 2014. They will need to be sampled again in 2020.

Well 1A was sampled in 2014. It will need to be sampled again in 2023.

ASBESTOS

In accordance with NYSDOH requirements, you are required to collect an asbestos sample at least once every nine years. Your last sample was collected on 8/19/13 and the results were below the MCL; therefore your next sample must be collected no later than 8/19/22. The sample must be collected at a tap served by asbestos cement pipe and under conditions where asbestos contamination is most likely to occur (i.e. at the end of the longest section of asbestos, in an area of aggressive water).
