RESPONSES TO QUESTIONS FOR SOLICITATION 36C24218Q0227:
1. The SOW lists testing frequencies that are specific to compliance with New York State requirements and far beyond those required by the VA and NHPP standards. Is the facility requesting testing frequency that is above what is required in a VA facility and that this minimum is all that will be accepted?
Answer: The requirement calls for equipment testing in compliance with New York State requirements, quarterly, semi-annually and annually.
2. Several statements speak to the re-testing of components after repair. Please clarify expectations for the statements below:
a. Page 6 of 63, 2.b) “Retesting of repaired or adjusted equipment within 30 days of the repair or adjustment. This testing may be limited to features and parameters that would be affected by the repairs or modifications. In order to maintain functionality and minimize the effect on patient care the Contractor shall be available to provide testing within four (4) hours.”
Answer: Testing is generally required within 30 days, however, at the VA’s discretion, testing may be required within four (4) hours.
b. Page 7 of 63, 5. b) “The qualified Diagnostic Medical Physicist shall perform a follow-up inspection to verify compliance with any necessary corrective action required to correct deficiencies found.”
Answer: If the Diagnostic Medical Physicist (DMP) has identified a deficiency, and the VA has since addressed that deficiency with corrective action, the VA may request a follow-up inspection by the DMP to verify the adequacy of the corrective action taken.
c. Page 9 of 63, 2. “Testing is required within 30 days of repair or modification to equipment and shall be requested by the VAWNYHS.”
Answer: Testing is generally required within 30 days, however, at the VA’s discretion, testing may be required within a shorter timeframe.
d. Page 9 of 63, 2. a) “In order to maintain functionality and minimize the effect on patient care, the Contractor shall be able to provide testing within four (4) hours, upon request.”
Answer: Testing is generally required within 30 days, however, at the VA’s discretion, testing may be required within four (4) hours.
3. It is clear enough that we would retest anything that is repaired within 30 days. This is typically something that can be scheduled well in advance for the convenience of the facility and could even be done before or after clinical work hours. The statement regarding the 4 hours are unclear. Does this mean that the re-test must be done, within a 4-hour window on the day that it is scheduled, with the scheduling being done in advance? Please be specific of the expectation.
Answer: Testing is generally required within 30 days and scheduled in advance, however, at the VA’s discretion and in consideration of patient care or other issues that may arise; testing may be required within four (4) hours of request.
4. If the offeror is teaming with a sub-contractor to perform services, will you accept the subcontractors past performance and references in addition to the primary contractor’s?
Answer: The prime contractor must meet performance and reference requirements of the solicitation.
If subcontracting is considered, performance and references of the proposed subcontractor may be submitted. It is important to note Section B.2 Limitations on Subcontracting of this RFQ solicitation. If the offeror intends to subcontract, the offeror will be required to adhere to FAR 52.219-14 and provide details how the offeror will comply with the requirement at least 50 percent of the cost of contract performance incurred for personnel shall be expended for employees of the concern.
5. What is the file size limitation for emailing a proposal to Michelle.Harsch@va.gov?
Answer: 5 MB
6. If a proposal has file sizes that exceed the limitation for one email, and it needs to be emailed in 2 or more emails, what do you want typed in the subject line?
