About this file

This document is a Quality Assurance Surveillance Plan (QASP) for Reference Laboratory Services for the Department of Veterans Affairs VISN 1 Consolidated Laboratory. The QASP outlines performance standards and surveillance methods for a laboratory services contract, including key tasks such as clinical information return, quality care, and turnaround time. The plan establishes seven primary performance indicators with specific acceptable quality levels (AQLs), ranging from 95-100% compliance, covering areas like consultative report generation, specimen handling, patient identification, licensing and accreditation, and delivery timeframes.

The surveillance methodology includes periodic inspections, random sampling, and contractor reporting, with a color-coded rating system (green, yellow, red) to evaluate contractor performance. The government will use the "Potential Exercise of Option Period" as an incentive, with the Contracting Officer's Representative (COR) responsible for assessing and documenting performance using Quality Assurance Worksheets. The document is designed to be a "living document" that can be reviewed and revised, with both the contractor's Program Manager and COR expected to sign the final version after contract award.
