STATEMENT OF WORK

VISN 1 NETWORK CONSOLIDATED LABORATORY

VA CONNECTICUT HEALTHCARE SYSTEM

West Haven Campus, Pathology & Laboratory Medicine Service Virology and Molecular Diagnostics Department

VISN 1 Network Consolidated Laboratory is requesting test kits and other ancillaries for VA Connecticut HCS, P&LMS, Virology and Molecular Diagnostics Departments required for their specialized testing.

This request is for a 5-Year Blanket Purchase Agreement with a period of performance: October 1, 2020 through September 31, 2025.

Virology Department:

1. The QIAsymphony DSP Virus/Pathogen kit enables automated purification of viral nucleic acids and bacterial/pathogen DNA from a broad range of sample materials for in vitro diagnostic use. Kits are available in mini and midi format for different sample volumes.

2. EZ1 Virus Mini kit v2.0 provides a fully automated procedure for simultaneous purification of viral DNA and RNA from serum, plasma, CSF, urine, respiratory samples and other cell-free body fluids for sensitive analytical detection in downstream assays. The kit provides optimized binding conditions for more robust and reproducible capture of nucleic acids, and improved wash conditions for higher analytical sensitivity in downstream assays.

3. QIAamp Viral RNA mini kit simplifies purification of viral RNA from cell-free body fluids with fast spin-column or vacuum procedures. Viral RNA binds specifically to the QIAamp silica membrane, and pure viral RNA is eluted in either water or a buffer provided with the kit. Purification can be fully automated.

This testing with Qiagen products has been validated for use and performed at the VA CT Virology Lab at the West Haven Campus for quite some time and serves as the VISN 1 Reference Lab providing results to other Labs within our VISN.

Molecular Diagnostics Department:

4. The Qiagen-Ipsogen JAK2 Mutation Testing, Qiagen EZ1 Virus Mini kit v2.0, and EZ1 DNA Blood 200uL nucleic acid extractions are to be used on multiple platforms in the Molecular Diagnostics Lab. The specialized testing performed by the Molecular Lab includes the JAK2 assay, HCV Genotyping, RBC Genotyping, and CYP2C19 Genotyping Assays. Testing platforms currently in use that perform this testing are the Veriti PCR System, the QuantStudio Real-Time PCR, the Immucor AIS System, the Siemens Autoblot 3000H automated platform and the GenMark eSensor platform.

This testing with Qiagen products has been validated for use and performed at the VA CT Molecular Diagnostics Lab at West Haven Campus for quite some time and serves as the VISN 1 Reference Lab providing results to other Labs within our VISN.

B.9 PRICING DATA

1. The prices included on the BPA list (or applicable "discounted" rates submitted in a proposal response to an RFQ resulting in award) that are in effect on the effective date of an order shall govern that order's basic performance period. With regard to any option years, which are later exercised, the proposed option year rates as incorporated into the order award are in effect until such time, if applicable, that the contractor has been authorized a rate increase culminating from a negotiation with the Contracting Officer.

Authorization for individual orders above the stated order and/or Ceiling Limits must be coordinated through the Contracting Officer before larger valued orders can be issued and prior to commencement of work. All unauthorized work, regardless of amount, will be processed through the ratification process.

B.12 OPTION TO EXTEND THE TERM OF THE BPA

a. The Government may extend the term of this BPA by written notice to the Contractor at any time prior to the expiration of the BPA, provided that the Government shall give the Contractor a preliminary written notice of its intent to extend at least days before the BPA expires. The preliminary notice does not commit the Government to an extension.

b. If the Government exercises this option, the extended BPA shall be considered to include this option provision.

B.13 GENERAL SUPPLY ORDERS

SECTION C - CONTRACT CLAUSES

C.1 BPA ORDER INTRODUCTORY LANGUAGE

The terms and conditions of the contractor's BPA (including any agreement modifications) apply to task or delivery orders issued under the Blanket Purchase Agreement (BPA), otherwise called the agreement, as a result of this RFQ.
