New Onsite Screening Process with Temperature Checks

1. Contract screeners will Initiate screening questionnaire at Main Hospital and Clinic entrances.

VA employees and or contractors should use the new COVID-19 Screening Tool and have the answers on their phones.

COViD-19 screening tool. The screening tool is an app intended to be the last line of screening. To prevent inadvertently exposing others, employees should first assess themselves for symptoms prior to coming to work. Employees who are feeling ill and experiencing symptoms associated with COVID-19 (see list of symptoms below) should not come to work. Employees who are experiencing signs or symptoms of illness should notify their immediate supervisor/service chief and personal healthcare provider by phone. Please note the paper screening tool being placed on your dash will be phased out. Once fully implemented, employees must access the app daily for self-assessment and get an entry validation for the day to show to the screeners:

COVID-19 symptoms may include, but are not limited to:

· Fever (100 degrees Fahrenheit or more)

· Cough

· Shortness of breath

· Muscle aches

· Malaise or flu-like symptoms

· Unexplained new diarrhea, nausea or vomiting

· New onset of unexplained neurologic symptoms, including headache or loss of taste or smell

For Staff: After showing the VA COVID-19 tool to proceed, a temperature check will be performed.

NOTE: If you do not have a smart phone and cannot access the application, you will be asked the screening questions.

Screening Questions:

Q. Are you a VA employee or contractor or Veteran here for an appointment?

Q. In the past 24 hours have you had a fever Q. In the past 7 days, have you had a cough, shortness of breath, or difficulty breathing that is new or getting worse?

Q. In the past 3 days, have you had any of these symptoms?

· Fever or feeling feverish (chills, sweating)

· Fatigue (felling tired all the time)

· Muscle or body aches

· Headache

· New loss of smell or taste

· Sore throat

· Nausea, vomiting, or diarrhea Q. Do you currently have a runny nose or congestion that is new and not related to Allergies?

Q. Have you had contact with someone you know was diagnosed with COVID-19 or was waiting for COVID-19 test results?

If the answer to the above questions shows negative screen, a temperature check will be performed.

Any person, staff or Veteran, refusing to have their temperature taken will be immediately directed to leave the premises.

If temperature is within normal range of 98 -100, employees will receive a color sticker based on the day of the week that will have the date stamped on it and allowed to enter the premises. This will be used to travel throughout the Temple campus without being rescreened for the day.
