About this file
This document contains Questions and Responses for a Request for Information (RFI) issued by the Louisiana Department of Health, Office of Aging and Adult Services (OAAS), regarding Program of All-Inclusive Care for the Elderly (PACE) expansion into underserved areas of Louisiana. The RFI seeks vendor information for developing and planning PACE services in areas not currently served, with responses due September 4, 2026. PACE is a Medicare and Medicaid program providing comprehensive medical and social services to individuals aged 55 and older requiring nursing home-level care while remaining in their communities. Louisiana currently operates PACE programs in the Greater New Orleans area, Greater Lafayette area, East Baton Rouge Parish, West Baton Rouge Parish, Avoyelles Parish, Rapides Parish, and has an expansion pending in the greater Shreveport area. Respondents must address proposed PACE center locations, number and timing of centers, strategies for serving rural areas as defined by the Rural Health Transformation Hub, and capacity to serve nursing facility residents transitioning to community settings, including those with serious mental illness.
The RFI is informational in nature and is not scored; there are no review criteria, and it does not constitute a formal solicitation or guarantee of future contract awards. Respondents may submit information about multiple service areas within a single application. Louisiana PACE organizations currently operate under enrollment caps of 200-210 participants. The RFI requires that proposed service areas include rural areas as defined by Louisiana's Rural Health Transformation Program, though no minimum number of rural zip codes is specified. Submissions must include organizational mission and history, three years of financial statements with five-year projections, marketing and outreach plans, infrastructure and start-up costs, specific building locations, experience operating existing PACE facilities, anticipated challenges, and affordable housing experience. Funding for PACE expansion may be sourced through Louisiana's Rural Health Transformation grant. All submissions must be electronic and directed to Jeanne Abadie, PACE Program Manager, at OAASContracts@la.gov with "PACE RFI" in the subject line.

PACE RFI Questions and Responses July 2026 (Responses due on September 4, 2026)
1. Is it possible to submit multiple service areas that we are interested in serving? And if this is possible, should we create multiple submissions that each speak to the one service area, or could we include multiple services areas in one application?
Response: Since this is an RFI, a single submission with information about any and all areas within Louisiana that could be well-served by PACE would prove helpful to us. Also, an indication as to why those areas are important to serve would be good information.
2. We understand that the RFI is open ended for any areas not currently served or awarded for PACE. We see the service areas listed on the LDH website for the existing PACE programs. Could you please provide the anticipated zip codes for the Shreveport award?
Response: A PACE organization is currently preparing an application to CMS to provide services to the Shreveport area. The zip code areas will be part of that application. At this point, the areas that will be served by that PACE organization have not been determined.
3. How many awards is LDH contemplating awarding based on this RFI?
Response: This is an RFI. An RFI is informational gathering, and it should not be confused with an RFP.
4. What are the review criteria for this RFI?
Response: This is an RFI. It is not scored. There is no review criteria.
5. Are there any enrollment caps that should be assumed as part of this RFI?
Response: Louisiana’s PACE organizations do have enrollments caps that fall between 200 – 210 participants.
6. We see that the RFI requires a rural area, as defined by the Rural Health Transformation Hub, as part of the service area.
a. Are there a minimum number of zip codes that must be included that are considered rural to qualify?
Response: This is an RFI. Louisiana would like to gather information regarding what rural areas could be best served by a PACE organization and why.
b. Is there a rural definition that should be referenced? The Hub website compiles multiple different definitions. Would it suffice if one of the definitions on that website indicates the region as rural?
Response: To understand more about how Louisiana is defining rural, I would suggest looking at Louisiana’s Rural Health Transformation Program website. It can be found here: Rural Health Transformation Program | Louisiana Department of Health
7. Could you please explain more what “incentive funding” is available through Louisiana’s Rural Health Transformation grant? How much is available and for how long?
Response: PACE is an identified service in Louisiana Rural Health Transformation Program, so Louisiana will entertain using monies from that grant to assure that PACE is provided in rural areas.
