STATEMENT OF WORK

Monthly Pest Control Services

1. Introduction: This statement of work describes the requirements for an outside contractor to provide Monthly Pest Control Services to NNMC Hospital, Sanostee Clinic, Toadlena Clinic, and Quarter Maintenance Compound at Northern Navajo Medical Center.

1.1 Place of performance: Worksite is in the Indian Health Service Health Center- Northern Navajo Medical Center, located on US Highway 491 North Shiprock NM.

1.2 Background: Indian Health Services (HIS) is an agency within the US Dept. of Health and Human Services and is responsible for providing Federal Health Services to American Indians and Alaskan Natives. The Provision of Health Services to members of Federal recognized tribes grew out of the special Government to Government relationship between the Federal Government and Indian Tribes. HIS is the principal Federal health care provider and health advocate for the Indian people. The goal is to assure that comprehensive, culturally acceptable personal and public health services are available and accessible to American Indian and Alaskan Natives. HIS currently provides health services to approximately 1.5 million American Indians and Alaskan Natives who belong to one of the 557 Federal recognized tribes in 34 states.

1.3 Scope: The Contractor shall provide Monthly Pest Control Services:

• NNMC Hospital (256,058 SqFt) Interior and Exterior Pest Control Service

• Sanostee Clinic (2528 SqFt) Interior and Exterior Pest Control Service

• Toadlena Clinic (1262 SqFt) Interior and Exterior Pest Control Service

• Quarter Maintenance Support Buildings, Interior and Exterior Pest

Control Service 2021 (1-8), 2022 (1-8), 2023 (1-8), 3000, 3001, 3002.

• Quaurter Maintenance Housing Interior and Exterior Pest Control Service

2001 to 2020, 2032 to 2035, and 1028 to 1211.

• Contactor shall be responsible for service reports and final invoice in IPP

(invoice processing platform) system.

1.4 The Contractor shall provide all labor, tools, necessary pest control supplies, transportation, equipment, supervision and all other necessary items to complete monthly on-site pest control services for Shiprock Service Unit Northern Navajo Medical Center. The contractor will also provide all documentation required by the Joint Commission on Healthcare Organization’s Environment of Care standards for Hazardous Materials and Waste.

1.5 The goal of this contract is to develop an integrated pest management system as described by the American Society for Healthcare Environment Services (ASHES) publication “ASHES Recommended Practice Series: Integrated Pest Management.”

All questions concerning the contract objectives and requirements are to be addressed through the Facility Maintenance Contracting Officer Representative.

1.6 Period of Performance: 01/01/2024 to 12/31/2024

2. Service Delivery Summary:

2.1 The contractor shall survey the buildings and grounds at least monthly to determine what pest control services are required. Pest control actions shall include the application of pesticides and/or herbicides and trapping as necessary to prevent/remove the identified pest. The contractor is also expected to recommend any alterations to the facility that would decrease the likelihood of pest problems, such as altering drainage systems, relocating lighting or sealing openings into the building, occasional invaders, such as pill bugs, sow bugs, earwigs, etc.

2.2 Pests to be controlled include insects, occasional invaders, such as pill bugs, sow bugs, earwigs, etc. are included, rodents, vertebrates, vegetation, birds (NNMC Hospital Only) 4 bait station on roof, and reptiles that are disease vectors or nuisances or are harmful to humans and their surroundings. The most common flies. The contractor’s vehicles shall be clearly identifiable as to their purpose and what EPA registered chemicals on board. Prior to any on-site work, the contractor’s personnel will sign in at the Facilities Management Office or Shop.

The contractor will also be required to sign in at the Security Office to obtain and possess a Contractor’s Badge while on the HIS premises. The contractor will only bring EPA registered chemicals on-site that have approved for use by the Project Officer. The contractor must have in their possession a Safety Data Sheet (SDS) for the chemicals being used and filled at the maintenance shop.

2.3 The work will either be scheduled on regular days each month or the contractor will provide at least 5 days of advance notice prior to any site work that would involve applications of any EPA registered chemicals. Emergency calls will need to be address day of or the day after.

2.4 All work will be performed at the Hospital will be done during normal business hours (8:00am to 4:30pm, Monday to Friday). No work will be scheduled during any holiday recognized by the Federal Government. If work must be performed during non-business hours, advance notification must be made through the Contracting Officer Representative who will notify the Contracting Officer for approval.

2.5 Every effort must be made to ensure that pesticides do not become hazardous waste. EPA registered pesticides cannot be stored anywhere on the NNMC compound by and/or for, the contractor. Also, All EPA registered pesticides and herbicides must be pre-approved in writing by the NNMC Contracting Officer Representative prior to being brought onto site. A sample of the pre-approved form that must be on file prior to the use of the EPA registered chemical is attached.

2.6 Work shall be accomplished in accordance with applicable federal, state, regulations and guidelines. The contractor is responsible for any damage caused by the contractor’s personnel or equipment to the NNMC site, buildings or equipment.

3. Government Furnished Property and Services:

3.1 Government will not supply labor, supplies, and equipment necessary to complete the work. All of the buildings on-site will be made accessible to the Contractor.

4. Contractor Licensing, Permits, and Personnel Qualification Requirements:

4.1 Permits: Not Applicable

4.2 Safety Programs: As required by OSHA and the hospital Safety Officer.

4.2.1 The Contractor shall maintain a comprehensive procedure and plans that the Contractor will implement to ensure safety and health of workers and visitors to the projects site, including setting up barricades, signs, and verbal warnings.

4.2.2 The Contractor shall have a list of major hazards associated with the work and shall identify in the safety program means to minimize the hazard.

The program shall specifically address all requirements of Occupational Safety and Health Act (OSHA) FCR 1926.1101.

4.2.3 The Contractor shall have a copy of the Safety Program at the work site and make available for the review to the workers upon request.

4.2.4 The Contractor shall be responsible for any and all damages that the Contractor Personnel cause to utilities, building, grounds, or any government property.

4.2.5 The Contractor shall conform to the Northern Navajo Medical Center Non-Smoking Policy; Patient Privacy Policy; and Non-Cellular Phone/Radio Usage Policy.

4.2.6 Work shall be performed in such a manner so that the pesticide and herbicide will be kept to a minimum due to clinic patients. The contractor will be working near patient treatment areas and dust and other contaminants must be contained.

4.2.7 Insurance Requirements:

The Contractor shall have valid and current liability insurance in force during the period of this contract.

4.2.8 Contractor and Personnel shall be licensed by the State of New Mexico:

Additionally, license shall specify those categories of pest control in which the license is qualified and authorized to perform pest control services.

As a minimum, all requirements with respect to business licensing, certifications, employee identifications, and actual work performance shall fully comply with applicable New Mexico regulations and guidelines.

5. Project Officer Responsibility:

5.1. The Contractor Representative (COR) shall:

A. Monitor the Contractor’s technical progress to include the surveillance and assessment of performance, B. Interpret scope of work.

C. Provide technical inspection & acceptance required by this Contract, D. Assist the Contractor in resolution of technical problem encountered during the performance of the contract.

6. Authorities/References:

6.1 Federal Insecticide, Fungicide, and Rodenticide Act (FIFRA) of 1972, as amended Resource Conservation and Recovery Act (RCRA) of 1976, as amended Federal Water Pollution Control Act of 1972, as amended by the Clean Water Act of 1977.

Emergency Planning and Right-to-Know Act (EPCRA) of 1986 Toxic Substances Control Act (TSCA) of 1976 Endangered Species Act (ESA) of 1973 Migratory Bird Treaty Act of 1918, as amended Federal Noxious Weed Act of 1974 National Environmental Policy Act (NEPA) of 1969

7. Definitions:

7.1 Acceptance: Constitute acknowledgement that the supplies and/or services conform to the applicable contract quality and quantity requirements.

7.2 Approval: Acknowledgement by the designated government employee that submitted, deliverable or administrative documents confirm to the contractual requirements.

Government approval does not relieve the Contractor from responsibility for compliance with contract requirement.

7.3 Contracting Officer (CO): A government employee with the authority to enter into, administer, and /or terminate contracts and male related determination and findings.

7.4 Contractor: The individual or company awarded a legal binding contract to provide supplies and services.

7.5 Project Officer: A federal employee who assists in ordering activity the contracting officer in the administration of task orders issued under this contract responsible for the technical assistance in day-to-day program management of the project activities

7.6 Federal Acquisition Regulation (FAR): The primary regulation for use by federal agencies in their acquisition of supplies and services with federally appropriated funds.

7.7 IPP (Invoice Processing Platform): Secure, web based, centralized program through which federal agencies better manage the invoicing process for goods and services, from purchase orders to payment notification.
