About this file

This is a required documents checklist and bidder certification package issued by the California Department of Veterans Affairs for Solicitation Number 25VS0006 regarding Water and Ice Machine Testing and Treatment Services. The solicitation requires bidders to submit a comprehensive package of documents including a signed Bid Rate Sheet, Payee Data Record (STD 204), Bidder Declaration (GSPD-05-105), Commercially Useful Function (CUF) Certification if applicable, and proof of valid California business licensing or corporate standing. Additional mandatory submissions include the Contractor Certification Clauses (CCC-4/2017), a sample invoice, and certificates of insurance naming the State of California as an additional insured. Bidders must certify compliance with nondiscrimination requirements, drug-free workplace standards, and applicable state and federal labor laws. The checklist specifies that all company names on submitted documents must match exactly with the company name on the Payee Data Record, and failure to submit required documents in the prescribed manner may delay contract award.

The solicitation includes specific certification requirements for Disabled Veteran Business Enterprises (DVBE) and California Small Businesses, with bidders required to declare their certification status and provide supporting documentation if applicable. Bidders using subcontractors must complete detailed subcontractor information tables, including verification of good standing, valid licenses, and California certification status where applicable. The document emphasizes that certified bidders, whether DVBE or small businesses, must provide a commercially useful function as defined under Military and Veterans Code Section 999 and Government Code Section 14837. Bidders must also certify they are not expatriate corporations, comply with sweatfree code requirements for applicable procurements, and meet workers' compensation and Americans with Disabilities Act compliance standards. The file includes a sample certificate of insurance template outlining required coverage types and limits, with the State of California named as an additional insured with respect to work performed.
