About this file

This document is a state government form related to the Darfur Contracting Act, which applies to companies with business activities or operations outside of the United States that are bidding on or submitting proposals for State of California contracts. The form provides two options for certification - either the company certifies that it is not a scrutinized company, or the company has received written permission from the Department of General Services to submit a bid or proposal as a scrutinized company. The form requires the company name, Federal ID number, authorized signature, and printed name and title of the signatory.

The document outlines the requirements for companies with foreign business activities to be eligible to bid on or submit proposals for State of California contracts. It cites the relevant Public Contract Code sections and specifies the certification process, including the option to obtain written permission from the Department of General Services on a case-by-case basis if the company is deemed a scrutinized company. The form must be completed and submitted with the company's bid or proposal.
