"Resolve", "Resolved", "Resolution" and correlative capitalized terms mean, with respect to any particular Support Request, that Contractor has corrected the Error that prompted that Support Request and that the State has confirmed such correction and its acceptance of it in writing. Contractor shall respond to and Resolve all Support Requests within the following times based on the State's designation of the severity of the associated Error, subject to the parties' written agreement to revise such designation after Contractor's investigation of the reported Error and consultation with the State. The State and the Vendor agree that the resolution plan for defects discovered in the Maintenance & support phase will be defined as assigning the individual defects to planned builds with target release dates. For defects in need of urgent remediation as determined by the State, the emergency release or hot-fix processes will be followed for resolution.
Warranty Clarifications from CN6 is replaced in its entirety with the following:

The defined warranty period of 120 days will be required for all software deliverables for both the Tax and Benefits releases for this Contract. The warranty period of 120 days will also be required for any software deliverables from either release, that is delayed until post go-live. The 120 days of warranty will start when each delayed software deliverable is released; however, both parties agree such warranty will not apply to code changes (including configurations) as part of M&O.
In the CN6 Approach to Testing table, the Exit Criteria clause for the “User Acceptance Testing” testing is replaced in its entirety with the following:
Exit Criteria:
100% of UAT test cases have been executed and a minimum of 95% of the executed UAT test scripts have passed and there are no open critical or high defects. The State must submit any exceptions to UAT Exit Criteria in writing to the Contractor and said exceptions must be approved by the Contractor in writing.

Item: 29 ADO #: 195156 Release: R2 Title: Misrepresentation adjudication - issue needs to open w/ parent issue
Description: Description of Change: MiUI needs to automatically open a misrepresentation case whenever a working & collecting matter is opened in order for fact-finding to go out simultaneously for both issues. Currently, MiUI only opens a misrepresentation -intentional issue AFTER you determine fraud on the parent issue (working & collecting). We cannot determine fraud on one issue and THEN ask for misrepresentation questions after the fact. Those questions & answers can be crucial when determining intentional vs. unintentional.
Reason for Change: MiDAS currently auto-opens both a working & collecting issue (not unemployed or remuneration) and an associated misrepresentation issue so both issues are closed at the same time. Additionally, the current set up in MiUI doesn't allow you to select a rationale for the intentional misrepresentation.

Description: Currently, MiUI uses the employer address from the IBIQ results in order to send the outgoing IB4 Request to states. The problem is that an employer might have an address in a state where wages are not assigned, resulting in IB4 Requests going to incorrect states and those states returning an unnecessary IB4 Response.
From ICON UAT Partner testing, a solution would be to use SIDI results for the IB4 instead of IBIQ. There would need to be a way for MiUI to account for CA, OK, and VI, which do not provide data to the Hub for SIDI inquiries.

Description: The current MiDAS file contains one record per customer. A meeting was held with WDC and DTMB staff and it was determined that the file containing multiple records, as it is currently designed for MiUI, is going to cause processing issues. If we keep the current MiUI IDD configuration with multiple records per customer;
Total Agility will need to make major changes on their side. In Midas; when we do the 6 year look back; we send a single SSN with the most recent claim information. MiUI configuration is sending ALL claims for the 6-year lookback. This would cause multiple SSN records for a single claimant and total agility would have to try to identify which BYB/Claim ID to use.
Summary: No changes to record layout. Report claimant data with most recent claim ID. Do not include additional records for each claimant with the additional claim IDs active in the 72-month lookback period.

Expanded UAT Scope
This CN accommodates a larger R2 UAT for the pre-CN10 scope than originally planned. The original scope for R2 UAT called for 3,198 test cases. Since R2 UAT began, an additional 6,264 test cases have been added for a total of 9,462 test cases.. This requires additional schedule time as well as additional resources from the Contractor.
New Hardening Phase
This CN adds a three-week hardening phase for R2 between the completion of R2 UAT and the R2 go-live. If additional hardening time is required, this will be mutually agreed upon via a change order. As with R1, the purpose of this phase is to allow the State to conduct additional, targeted testing of critical high-visibility functions with MiUI. It also allows the State to complete any UAT testing that the State has elected to move to the hardening phase, as allowable per the schedule.
Another component of hardening is testing and confirming the operational readiness of UIA staff to use MiUI (R2). Staff will perform a set of business operations with a focus on the high priority, high volume business processes. It’s important that Staff go through the operational validation process in order to encounter and understand where and when business processes, decisions and handoffs behave differently in MiUI vs MiDAS.
During hardening, the UIA Staff will validate business operational readiness through a simulation of executing key business processes from the various business units. For example:
• Simulate critical, high volume business processes such as filing initial claims, determine monetary eligibility, perform non-mon adjudication, process payments, collections etc.
• Utilize dashboards to monitor and manage staff activity daily while hardening is executed;
identify job aids as questions surface from staff on how to perform essential job duties. Decide how work will be completed, are they able to complete the work, are resources available if questions arise, do they know where and who to ask for help?
• Understand dependencies across teams (e.g., intake → monetary/non-monetary issues), where the handoffs occur between the different business units.
As with R1, there are no formal entry or exit criteria for this phase.
User Testing Phase for New CR’s
The 37 CR’s that are included in this CN add significant scope to R2, resulting in the need for a new User Testing phase dedicated to this new scope. The State needs to perform testing on these CR’s prior to the R2 go-live, requiring an additional 11 weeks of time. If additional User Testing time is required by the State, this will be mutually agreed upon via a change order. This includes additional support from the Vendor team as well as remediation of any issues that are found during testing.
The additional testing effort covers the following activities:
- Identify testing requirements for the 37 CR’s
- Write test cases to meet these testing requirements, planning for a mix of testing using converted data as well as data created by MiUI
- Execute test cases, log test case progress, record any issues, resolve issues, retest any failures
Any required entry or exit criteria for the User Testing phase will be developed by the project team and must be mutually acceptable to both the State and the Vendor.
Enhanced Outreach
This CN provides for additional outreach support from the Contractor for Employer, TPA, and Claimant outreach. This augments the State’s approach for Outreach and consists of:
1. Creating the MiUI Employer Toolkit
2. Additional Navigator tours for TPA’s
3. Provide open Q&A and one-on-one support for the State’s MiUI Office Hours
4. Create an additional 10 guides for training delivery
5. Deliver two 3-hour functionality deep dives for Employers and TPA’s
6. Support demonstrations and logistics for UIA division roadshows
Nine Months of Enhanced R1 Support to Accommodate Various Enhancements
The State has asked for additional capacity to implement changes to MiUI R1 (Tax) during the first nine months following the completion of R1 warranty (beginning 6/24/26). This continues to be a capacity-based model as defined in the contract. For the first nine months of M&O, the M&O team size is adjusted to the following levels:
The staffing levels and payment schedule for the first nine months of M&O is adjusted as follows:
