About this file

This is a Vendor Authorization Request form for domestic vendors of ASRC Federal and its subsidiaries. The form establishes vendor eligibility and tax compliance for payment processing.

The form requires vendors to provide taxpayer identification information including either an Employer Identification Number (EIN), Social Security Number (SSN), or Individual Taxpayer Identification Number (ITIN). Vendors must supply legal name, business name or DBA, legal address, remittance/payment address, and contact information. Required identifiers include UEI number, CAGE code, and DUNS number if applicable. Vendors must select their entity type (individual/sole proprietor, corporation, partnership, trust, LLC, or other), entity classification for tax purposes, and business size (small business, large business, foreign owned, non-profit, or other). The form also requires disclosure of any applicable business classifications including SBA certifications (8(a), HUBZone, WOSB, service-disabled veteran-owned, disadvantaged business), and other designations such as historically black colleges and universities or Alaskan Native Corporations. ASRC Federal defaults to Net 45 payment terms for large businesses and Net 30 for small businesses unless otherwise stated in the purchase order. Vendors must enroll with Corpay Payment Automation for electronic payments after receiving one paper check. The form requires certification under penalty of perjury regarding correct taxpayer identification and backup withholding status. Authorized signatures must use certified digital signatures or wet ink only.

Vendor Authorization Request - Domestic Instructions PAGE 1

For ASRC Federal and Subsidiary Vendors ONLY

Instructions For General Instructions refer to the IRS Form W-9 located on the IRS website (www.irs.gov).

ASRC Federal defaults to Net 45 payment terms for large business and Net 30 for small business, unless stated otherwise in the sales contract/purchase order.

ASRC Federal and its Subsidiaries (ASRC) may award procurement to the seller where the costs will be charged to a U.S. government prime or subcontract.

If so, the seller is advised that the U.S. government may impose a penalty against a firm misrepresenting its business size and/or disadvantaged status for the purposes of obtaining procurement that is to be included as part, or all of a goal contained in ASRC’s Subcontracting plan. Eligibility as a small business is based on the regulations issued by the Small Business Administration in CFR 13, Part 121 of the SBA Rules and Regulations and FAR 52.219-1.

Electronic Payments ASRC Federal has partnered with Corpay Payment Automation to facilitate electronic payments to our Vendors. To receive electronic payments, you must enroll with Corpay Payment Automation Note: You are required to receive one paper check prior to enrolling in Corpay Payment Automation
