CN 4 – STATEMENT OF WORK - MDOC
Michigan Department of Corrections serves individuals in the criminal justice system that may have significant barriers to employment, rely on several social safety net programs for support and could benefit from receiving agency services while under supervision.
Services provided include, but are not limited to: assessments, vocational counseling and guidance, information and referral, vocational training, job related services, job placement, and employment retention supports.
Verifying employment would assist MDOC employment specialists in identifying employed clients, obtaining appropriate employment documentation, and providing additional support to those clients that are unemployed or underemployed.
MDOC will use the system statewide, with a focus on those clients that have completed job training or educational programs while incarcerated, allowing MDOC employment specialists to obtain employment verification for those that are subsequently released from prison. Access to the system will be useful to locate clients believed to be employed but where lack of communication and client contact has been difficult.
The Offender Success program would benefit by enabling the agency to accurately report competitive integrated employment outcomes. This reporting is necessary to comply with state statutory reporting requirements for the Department.

E. The Work Number Express Social Service Verification - Pricing Terms and Conditions.
• Transaction. A 'Transaction' is defined by a database search which returns data in the form of a Verification Report. A ‘Verification Report’ is the output file returned from a database search containing various employer records. Each employer returned in a Verification Report constitutes a separate Transaction. For example, two (2) employers returned in a Verification Report will count as two (2) separate Transactions.
• Order Date Ranges. The above pricing reflects the rate for each Transaction. The specific use(s) or program(s) shall be denoted by the Agency in each request. Agency may select which employment records to order when accessing the Service online by selecting from various “pay date” range or “Purchase all” options. Order options include: “3 full months,” “6 full months,” “1 full year,” “3 full years,” and “Purchase all” records. Date ranges go back in full calendar months, i.e. selecting “3 full months” on January 30, would result in all records available between October 1 and January 30. If multiple records exist in the date range option Agency selects, then each record will count as a separate Transaction.
• Overages and Annual Commitment. The Minimum Annual Block Price for the Service(s) listed above will be payable in 12 equal Monthly Fixed Fee installments. The Annual MultiVerify Subscription Service fee for the Service(s) listed above will be payable in 12 equal MultiVerify Monthly Fixed Fee installments. Agency agrees and acknowledges that the Monthly Fixed Fee and MultiVerify Monthly Fixed Fee will be due and payable even when no transactions are processed during the period. Prior to the Transactions in the block chosen being exhausted, Agency must purchase another block of Transactions to continue the Service. If another block of Transactions is not purchased prior to the then active block being exhausted, Agency shall be automatically enrolled in the Overage Block
Transactions Included at the Overage Block Price which shall be re-elected thereafter upon exhaustion of the then current block until the then effective Term has expired. The charge for such block(s) will be spread across the months remaining in the then contract Term. At the end of the Term or upon termination by Agency, if the Agency has not met the Block Transactions Included as required, Agency shall pay one hundred percent (100%) of the remaining Minimum Annual Block Price and Annual MultiVerify Subscription Service due under the then current year obligation. Should the Agency terminate prior to the end of the month for any reason other than a lack of appropriation or negative appropriation, that month’s Monthly Fixed Fee and Monthly MultiVerify Fixed Fee, along with any Overage Blocks then triggered and any unpaid invoices until that time, shall be considered part of the remaining balance for the Services offered and provided through the point of termination. All sums due as trigged under these circumstances will be paid as set forth in the Agreement.
• FNS EVS Program. Agency may participate in the FNS EVS Program for its eligible MultiVerify programs when it (1) maintains a valid agreement with EVS for use of the Services and Data; (2) has signed the MOU, including its Exhibit A, with FNS; and (3) specifics which programs will be utilizing a specific TWN report each time Data is requested. TWN transactions returned that qualify under the FNS EVS program requirements will be invoiced to FNS while those that do not satisfy those requirements will be involved directly to the Agnecy under this Agreement. The specific FNS EVS program requirements have been provided under separate cover, these requirements may change from time-to-time based upon FNS and program requirements.
F. Commercial Verification of Employment – Pricing Terms and Conditions.
• Contractor has provided pricing for the Commercial Verification of Employment specific to the Office of Michigan Guaranty Agency in support of collection activities. Approved use is limited to obtain employment on defaulted loan borrowers for the purpose of wage garnishment on borrowers that do not submit voluntary monthly payments.
