About this file
This is a Request for Quotation (RFQ) document for the procurement of Angio Mentor Flex equipment by the state of Washington. The RFQ seeks quotations for the supply and delivery of Angio Mentor Flex Pro 3.3 systems, which are advanced medical simulation devices used for angiography training. The document establishes the technical specifications and requirements for the equipment procurement, including detailed performance standards, system capabilities, and compatibility requirements. Specific quantities, response deadlines, and delivery timelines are outlined to guide potential vendors in submitting compliant quotations. The RFQ process includes provisions for site visits or technical demonstrations if required by bidders to assess system specifications and capabilities prior to proposal submission.
The pricing structure for this procurement requires vendors to provide competitive quotations based on the specified equipment configurations and delivery terms. The RFQ likely includes evaluation criteria weighting pricing competitiveness alongside technical capability and vendor qualifications. Washington State's procurement follows standard governmental purchasing procedures with defined evaluation timelines and award processes. Funding for this medical simulation equipment is provided through the state's budget allocation for healthcare training and educational infrastructure. The contract terms specify equipment delivery, installation support, warranty provisions, and post-delivery service obligations to ensure operational readiness of the Angio Mentor Flex systems upon receipt.

RFQ Specifications
I. DEVIATIONS AND ALTERATIONS
Deviations, exceptions, and alternate proposals are not permitted and will not be considered.
Respondents must submit responses that fully comply with the requirements of this RFQ. The College reserves the right to reject as non-responsive any response that includes deviations, exceptions, conditions, or alternates.
II. ERRORS
Unit prices will serve as the basis for the award and for correcting this order if an error is made by the bidder when entering or completing responses to this RFQ, including errors in computations, calculations, or extensions.
III. FREIGHT
Unless otherwise specified, prices are FOB Destination, freight prepaid and included.
IV. INSPECTIONS
Goods provided under this Contract are subject to inspection and testing by the Collage at times and locations determined by the College. If the College determines that the goods are incomplete or otherwise do not comply with the proposal specifications, the College may reject the goods and require the seller either to correct them at no additional cost or to provide them at a reduced price, whichever is most equitable under the circumstances. If the seller is unable or unwilling to correct the goods within a time frame the College considers reasonable, the College may cancel the order in whole or in part. Nothing in this paragraph limits or otherwise affects the College’s rights, including its rights and remedies under the Uniform Commercial Code.
V. DEVIATIONS / NON-CONFORMANCE
Any units that do not conform to the specifications and/or Statement of Work set forth in this RFQ may be rejected by the College. The successful respondent is responsible for complying with all requirements and for proposing or providing a solution that meets the specifications, capabilities, and performance requirements contained in this RFQ. Exceptions are permitted only where this RFQ expressly states that deviations will be considered and where the bidder has specifically identified the deviation and the College has accepted it.
VI. EVALUATION
The College’s designated end-user will evaluate the goods and work provided under this RFQ for a period of up to ninety (90) days following the delivery of goods or completion of performance. If the goods and/or work are determined to be unacceptable, the successful respondent shall either provide a replacement at no additional cost to the College or cancel the agreement and remove and return the goods at no additional cost to the College.
VII. PRICE INCLUSIVE
The price(s) offered in response to this RFQ shall include all costs and charges associated with boxing, packaging, and crating. The College will not allow any additional costs or charges for boxing, packing, or crating unless expressly authorized in writing by the College.
VIII. COMPLIANCE WITH LAWS AND REGULATIONS
By accepting an order resulting from this RFQ, the respondent warrants compliance with all applicable federal, state, and local laws and regulations and agrees to indemnify and defend the College against any loss, cost, liability, or damage arising from the respondent’s violation of this provision.
IX. COMPLIANCE WITH TERMS
The College may at any time insist upon strict compliance with these terms and conditions, notwithstanding any previous custom, practice, or course of dealing to the contrary.
X. AWARD FACTORS
Any contract awarded as a result of this RFQ will be awarded to the lowest responsive and responsible bidder, subject to applicable statutory preferences. Contract award decisions may be based on the considerations, requirements, and criteria set forth in RCW 39.26.160 or RCW 39.04.350, as further detailed in the Technical Specifications and Statement of Work sections of this
RFQ.
A. Notwithstanding any provision to the contrary in this RFQ, the College and its affiliates (Spokane Community College and Spokane Falls Community College), reserve the sole right to make multiple awards, make separate awards to different vendors for the College and/or its affiliates, or split an award among multiple respondents. Any contract award shall be non-exclusive. If deemed to be in the best interest of the College and its affiliates, awards may be made in any combination that best meets the needs of the College and its affiliates.
B. The following additional considerations, requirements, and criteria set forth in RCW 39.26.160(3):
a. Best value criteria including:
i. Whether the response satisfies the needs as specified in this RFQ, including whether the response meets the delivery date requirements of this RFQ.
C. The College may, at its sole discretion, reject the response of any respondent who has failed to perform satisfactorily on a previous contract with the College or the State of Washington per RCW 39.26.160(5).
D. Pursuant to RCW 39.26.160(6), after reviewing all submissions, the College may enter negotiations with the lowest responsive and responsible respondent to determine if the bid may be improved. An agency may not use this negotiation opportunity to permit a respondent to change a nonresponsive bid into a responsive bid.
E. Any items not specifically identified within this quote may be negotiated with successful respondent on an “as-needed” basis.
F. Quoted prices shall be all-inclusive. No additional payments will be made for fees, inspections, affidavits, permits, licenses, bonds, insurance, drawings, engineering, materials, labor, equipment, tools, shipping, travel, parking, or any other costs of any kind. This RFQ will be awarded on an “all-or-nothing” basis.
XI. COMPLAINT PROCESS
Pursuant to RCW 39.26.170, the complaint process is intended to provide respondents with an opportunity to identify and raise concerns regarding the solicitation requirements or evaluation process early enough for the College to address any issues before bids are submitted and evaluation efforts begin. Complaints may be submitted only by vendors that intend to respond to the solicitation.
Complaints received on the solicitation due date will be reviewed and considered only if time permits.
Complaints that do not comply with these procedures will not be considered.
The procurement complaint process will meet the following minimum requirements:
A. Respondents will have the opportunity to submit a complaint to the College based on any of the following criteria:
a. The solicitation unnecessarily restricts competition;
b. The solicitation evaluation or scoring process is unfair or flawed; or
c. The solicitation requirements are inadequate or insufficient to prepare a response.
B. Complaints must be submitted no later than five (5) business days prior to the bid response deadline. To be eligible for review, complaints are required to meet the following criteria:
a. Must be in writing;
b. Must be sent to the procurement coordinator, or designee;
c. Should clearly articulate the basis for the complaint; and
d. Should include a proposed remedy.
C. The procurement coordinator or designee will respond to all complaints in writing. In preparing the response, the procurement coordinator or designee will consider all available facts. The written response will include a decision, a description of how the review was conducted, and the basis for the decision.
D. The response to complaints, including any changes to the solicitation, will be posted as an amendment on the Washington Electronic Business Solution (hereinafter “WEBS”) online platform.
E. The Purchasing Manager will be notified of all complaints and provided with a copy of each corresponding response.
F. The complaint may not be raised against during the protest period.
G. The complaint process does not include an appeal process. This complaint procedure constitutes the sole administrative remedy available to vendors under this solicitation.
XII. PROTEST PROCEDURE
Pursuant to RCW 39.26.170, the protest process occurs after bid submission and evaluation. This procedure allows respondents the opportunity to raise concerns regarding the evaluation process and helps ensure its integrity and fairness. Protests may address issues related to the evaluation process described in the solicitation or the manner in which the process was conducted. The protest process provides an opportunity to identify and correct evaluation errors or other issues before a contract is executed. Protests that do not comply with these procedures will not be considered.
The protest process will meet the following requirements:
1. After the announcement of the apparent successful bidder (hereinafter “ASB”), respondents will be offered a debriefing upon written request.
2. Respondents will be given three (3) business days after the ASB is announced to request a debriefing.
3. Respondents are required to participate in a debriefing as a prerequisite for submitting a protest.
4. Respondents will be given five (5) business days after their debriefing to file a written protest.
5. The protest process will allow respondents an opportunity to submit a protest based only on the following:
A. A matter of bias, discrimination, or conflict of interest on the part of an evaluator;
B. Errors in computing the scores; or
C. Non-compliance with procedures described in the procurement document or the College’s protest process.
6. Protests are to be submitted to the College’s Purchasing Manager.
7. A written response will be provided within ten (10) business days from receipt of the protest, unless additional time is needed. The protesting respondent shall be notified if additional time is required.
8. The final determination of the protest shall:
A. Find the protest lacking in merit and uphold the College’s action;
B. Find only technical or harmless errors in the College’s acquisition process and determine the College to be in substantial compliance and reject the protest; or
C. Find merit in the protest and recommend options to the College, including:
1. Correction of errors and re-evaluation of all proposals;
2. Reissuance of the solicitation document, followed by the commencement of a new procurement process; or
3. Making additional findings and taking other actions as appropriate.
9. The protest decision is final. If a protesting respondent does not accept the protest response, the respondent may try to seek relief from the superior court. If the College determines that the protest is without merit, the College will enter a contract with the successful vendor. If the protest is determined to have merit, one of the alternatives noted in the preceding paragraph will be taken.
Upon receipt of the protest, a protest review will be held by the College. The College’s Purchasing Manager or an employee delegated by the Purchasing Manager who was not involved in the procurement will consider the record and all available facts and issue a decision within five (5) business days of receipt of the protest. If additional time is required, the protesting party will be notified of the delay.
In the event a protest may affect the interest of another vendor that also submitted a proposal, such vendor will be given an opportunity to submit its views and any relevant information on the protest to the RFQ coordinator.
