Section C - Description/Specification/Work Statement
C.1 Background and Need
In 2011, Lyme and other tickborne diseases (TBDs) were collectively the 2nd and 3rd most common nationally notifiable diseases in the New England and Mid-Atlantic states, respectively, with over 25,000 confirmed and probable cases reported in these regions. Tickborne diseases in these areas are primarily diseases cycling among animals. In general, larval ticks may acquire pathogens by feeding on infected rodents (particularly the white footed mouse, Peromyscus leucopus). Infections are retained as larvae become nymphs, and as nymphs become adult ticks. Humans acquire tickborne diseases incidentally, most often through the bite of infected nymphs.
One of the most promising options for prevention of TBDs is to reduce ticks on rodent hosts in the peridomestic environment. Rodent bait boxes have been developed for this purpose and are targeted to white-footed mice, as well as voles, chipmunks, and insectivores, such as shrews. Bait boxes contain wicks loaded with a low-dose acaricide (trade name: fipronil), which can provide protection against ticks for up to seven (7) weeks. In a field trial on Mason's Island, Connecticut, these boxes reduced populations of questing nymphal Ixodes scapularis by 68% in treated areas and also reduced the infection rate of these ticks for Borrelia burgdorferi and Anaplasma phagocytophilum (Piesman, 2006; Dolan et al., 2004). By targeting immature ticks on rodents, bait boxes have a substantial and lasting effect in the peridomestic environment (Schulze et al., 2007).
Bait boxes must be placed in yards for at least two consecutive seasons to be maximally effective. This allows for the intervention to target ticks at multiple life stages to more rapidly reduce tick populations and, thereby, impact pathogen transmission cycles. The first round of bait boxes should be deployed in late summer (July through October) to kill larval ticks on rodents. This will prevent many larval ticks from becoming human biting nymphs and egg-laying adult ticks in the next year. A second round of bait boxes should be deployed the following spring (May through mid-July) to target nymphal ticks feeding on rodents. This will again kill ticks that would eventually molt into adults that lay eggs. As with the first round, a third round of bait boxes should be deployed in late summer (July through October) to again target any remaining larval ticks on rodents. By the third year, this sequential effort should substantially impact total tick populations (questing or otherwise) on treated properties.
However, the level of protection against Lyme disease or other TBDs acquired by humans that is afforded by active bait boxes is not known. General acceptance of this prevention method may be improved by evidence-based knowledge of the level of efficacy. Therefore, CDC branches working on tickborne diseases, in collaboration with the Connecticut Emerging Infections Program (CT EIP) site, began the pilot phase of the Lyme and Other Tickborne Diseases Prevention Study (LTDPS) Bait Box Intervention (BBI) in 2012 in several counties in Connecticut that are highly endemic for Lyme disease. This randomized, single-blinded, placebo-controlled trial is designed to evaluate TBD underreporting and the efficacy of fipronil-containing rodent bait boxes to reduce the occurrence of TBDs (Lyme disease, anaplasmosis, ehrlichiosis, and babesiosis). The pilot phase of this study is being conducted under another contract.
C.2 Project Objectives
The purpose of this procurement is to enable a pest control company to conduct repeat seasonal placements and removals of commercial bait boxes on residential properties in Connecticut for 500 households. Fairfield and Litchfield Counties, both highly endemic for Lyme disease, will be selected for this study. These households will be randomized to receive either fipronil-containing bait boxes (up to 260 households) or untreated, placebo bait boxes (up to 260 households). The exact number of residential properties will depend on the number of households successfully enrolled into the study, but the enrollment goal of 500 households is to ensure sufficient power to detect any significant differences between treated and untreated (i.e., control) households.
The benefits of this research to public health include determining the efficacy of these commercially available bait boxes in preventing human tickborne diseases, thereby allowing CDC to make evidence-based public health recommendations on the most effective TBD prevention methods available. If these bait boxes prove effective in preventing disease, use of the boxes by the public may reduce TBD incidence in endemic areas.
C.3 Scope of Work
The scope of work pertains to a non-severable contract aimed at providing pest control services as part of the LTDPS (BBI) as described above. In brief, these services will involve repeat seasonal placements and removals of commercial bait boxes (Maxforce, TMS, Bayer Environmental Sciences, Inc., Montvale, NJ) on residential properties in Connecticut for 500 households during select time periods: first placement (on or before 31 July 2013) and removal (on or before 31 October 2013); second placement (on or before 1 May 2014) and removal (on or before 1 July 2014); third placement (on or before July 2014) and removal (on or before 31 October 2014). Households in Fairfield and Litchfield Counties in Connecticut will be randomized to receive either fipronil-containing bait boxes (up to 260 households) or untreated, placebo bait boxes (up to 260 households). Active bait boxes will contain a wick designed to passively treat rodents with fipronil, and placebo bait boxes will contain an untreated wick or no wick. The exact number of residential properties will depend on the number of households successfully enrolled into the study. Enrollment and provision of enrollees’ contact information is the responsibility of CDC and CT EIP; it is not the responsibility of the commercial pest control company.
C.4 Technical Requirements
1. Proper state-specific and/or county-specific pest control operator licensure (i.e., Certified Select Tick
Control System, TCS, installer) is required to install commercial bait boxes (Maxforce, TMS, Bayer Environmental Sciences, Inc., Montvale, NJ) in Fairfield and Litchfield counties in Connecticut. A scanned copy of the licensure should be provided to CDC and CT EIP study coordinators via email (Shook@cdc.gov, sara.niesobecki@yale.edu).
2. Consistency is required for all study treatments, per the study protocol. An outline of the proposed training method should be detailed within the submitted proposal. CT EIP shall participate in the training session and communicate with the contractor’s regional and/or local branch offices, as needed. The training shall be tailored to LTDPS-specific study requirements, as outlined in the LTDPS BBI protocol.
These requirements include that pest control operators (PCOs) shall prevent disclosure of the box type received by the LTDPS participant.
CT EIP will conduct a follow-up, in-person, site-specific LTDPS BBI training for staff (branch managers, office support, PCOs etc.) to be involved with property treatment scheduling, installations, and data entry. All staff shall attend this training. Additionally, all PCOs participating in this effort shall mailto:Shook@cdc.gov mailto:sara.niesobecki@yale.edu complete a pest control commercial training and certification program, which covers treatment techniques that conform to regulatory standards, set by NSF Cook & Thurber, Silliker, SQF, etc., prior to training for industry best practices. PCOs must be up-to-date on the latest pest management techniques, as they relate to tick management services.
3. CT EIP will provide the contractor with a list of 500 study enrollees eligible for property treatments.
Information from this list will be used or referenced during communications with CDC and CT EIP regarding: bait box installation; scheduling (and rescheduling as needed) LTDPS participant property treatments (i.e., bait box placements and removals); managing appropriate scheduling documentation;
completing participant reminder phone calls or emails for scheduled property treatments at least forty-eight (48) hours in advance of treatment date; and providing schedule and property treatment information to CT EIP on a periodic basis, as requested.
Note: For property treatment scheduling and reminders, LTDPS BBI participant contact information shall remain confidential, such that participant contact information is not released to other participants during communications. The contractor shall use the blind carbon copy email feature for all LTPDS BBI participant mass email communications.
4. All residential properties shall be inspected by the contractor to ensure they conform to the LTDPS BBI pre-treatment preparation checklist prior to initiating the property treatments. This pre-treatment preparation checklist is included in the participant’s enrollment packet, and a copy of this checklist will be provided to the Contractor in advance. The study protocol stipulates that three (3) bait box placements and three (3) bait box removals be performed per property for 500 properties. The exact number of residential properties will depend on the number of households successfully enrolled into the study.
Bait box installments shall be performed according to the following timeline: first placement (on or before 31 July 2013) and removal (on or before 31 October 2013); second placement (on or before 1 May 2014) and removal (on or before 31 July 2014); third placement (on or before July 2014) and removal (on or before October 2014). Half of the properties will receive three (3) consecutive installments of active, fipronil-containing bait boxes and the other half will receive three (3) consecutive installments of untreated, placebo boxes. The number of bait boxes to be used per property will depend on the treatment group (i.e., households with active vs. placebo bait boxes), property size, and presence of suitable mouse habitat. Placebo bait boxes should be spaced approximately 20-25 meters apart. The active bait boxes should be spaced ten (10) meters apart, per manufacturer’s recommendations. Most properties will be approximately two (2) acres (range: 0.5 to 5 acres). For this acreage size, it is estimated that 8-10 placebo bait boxes and 16-20 active bait boxes will be needed per property. All boxes should be placed in the property ecotone (i.e., at the interface of maintained landscaping/lawn with woodlot, brush, or stonewall).
LTDPS study staff shall observe the first 1-2 weeks of property treatments to confirm proper procedures outlined in the LTDPS BBI trainings are being followed. The study staff may also choose to observe other select treatments throughout the study treatment period. LTDPS BBI participants are not required to be present during their property treatments. However, PCOs will approach each homeowner and initiate communication to confirm treatment address prior to treatment. If the participant is present, PCOs shall not disclose the specific treatment applied (active vs. placebo bait boxes) under any circumstances.
5. The Contractor shall provide invoices after each service rendered (after LTDPS BBI-specific training for staff, after each bait box placement has been completed for all study participants according to the time line list above and in deliverable #5, and after each bait box removal has been completed for all study participants according to the time line list above and in deliverable #5). Invoices shall be directed to the appropriate office/point-of-contact at CDC’s Financial Management Office (FMO). Contractor shall send invoices to FMO, the Contracting Officer’s Representative (COR), and the Contract Specialist in accordance with clause CDCAG001 – Invoice Submission (March 2006). For invoices for either installments or removals, a line item for the number of properties serviced and the average cost per property for these services will be included.
6. CDC and CT EIP will require service updates from the Contractor throughout the course of the contract performance period. These service updates are necessary for CDC and CT EIP for data cleaning and validation efforts, which would involve direct communication with and multiple data transfers to CT EIP, as requested. The Contractor may be asked to provide and/or verify property treatment data such as treatment dates; confirmation of completed property treatments; locations of property treatments; types of property treatments (active vs. placebo bait boxes); property characteristics; number of bait boxes placed; etc. An increased need for these services is anticipated towards the end of the contract performance period during the study analysis phase.
Contractor shall provide a final report verifying property treatment data such as dates and confirmation of completed property treatments; locations of property treatments; types of property treatments; property characteristics; number of bait boxes placed; etc. to CDC and CT EIP study coordinators via email (Shook@cdc.gov, sara.niesobecki@yale.edu).
C.5 Work Breakdown Structure
C.6 Reporting Schedule
The period of performance shall commence on May 1, 2013 and shall continue through the end of the calendar year, December 31, 2014. Due to the nature of services to be rendered, Contractor will not be responsible for providing formal progress reports of completed deliverables to CDC.
C.7 Special Considerations
Contractor Access to Sensitive Information: Contractor will not have access to LTDPS BBI participant-sensitive information (e.g., health history, family relationships, etc.). However, the Contractor will have access to participants’ personally identifiable information (PII) such as name, address, and phone number, as this is necessary for completing the required deliverables. The use of participants’ PII is for the installation and removal of the bait boxes only and shall not be used for other purposes. This PII shall be stored in a secure manner while in use by the Contractor, according to the Contractor’s own proprietary computer security protocols, such that only the President the organization and those pest control operators who will be
