About this file

The document appears to be a proposal response questionnaire from Koski Construction, specifically a response form for a potential state and local contract. The document includes details about contract signature authority, with Sarah Koski Noufer identified as the Vice President, whose contact email is sarah@koskiconstructionco.com. The questionnaire seems to be part of a bidding process for a potential contract, likely related to construction or infrastructure services.

The document indicates that the proposal requires several key submissions, including a completed Pricing Page, an Affirmation & Disclosure Form, and potential attachments. While specific contract details are limited in this excerpt, the questionnaire suggests a structured procurement process with requirements for document uploads, signature authority verification, and compliance documentation. The form appears to be for a potential contract in the transportation or infrastructure sector, specifically referencing a code "101G-26 Asphalt Concrete, Bitum", which implies potential work related to asphalt or concrete construction services.

INSTRUCTIONS
English
You can use this file to create and/or update your questionnaire.
Important: This file's format is specific to the questionnaire type it is associated with and cannot be used for another type.
This file contains two data tabs: Questions and Constraints.
*Questions tab: Use this tab to add your questionnaire's sections, sub-sections, and questions.
*Constraints tab: Use this tab to set up conditional behaviors based on respondents' answers.
Questions Tab
An easy way to figure out how to create and set up the various types of questionnaire elements (sections, subsections, and questions) is to create a test item of each type, and then download the MS Excel® file.
In most cases, column values must be selected from a predefined list. This list can be accessed by clicking the cell's down arrow (lower right corner).
In the case of Multiple Choice questions, you must provide the list of possible answers and associated scoring points.
*For a non scored multiple choice question, enter possible answers in the "Choices for answer" column and speficy an associated score of zero for each answer, like so:
Yes=0

No=0

*For a scored multiple choice question, enter possible answers in the "Choices for scoring" column and speficy each answer's associated score, like so:
Audited & Certified quality system, good maintenance=10

Audited & Certified quality system=7 Non-certified Quality system=5 No quality system, but steps are taken=2 No quality system, or missing certificates to back supplier's claims=0 If your questionnaire includes at least one section, then all questions must be attached to a section.

Constraints Tab
You have the abillity to set up conditional behaviors. For instance, you can choose to include some questions only if respondents select a given response, or to make a question, a comment, or a file attachment mandatory, all based on respondents’ answers.
Here is an example to illustrate how to set up a conditional behavior. Let's say you have 2 questions, both located in the same section called "Documentation":
Q14 – Do you provide a manual?
Q15 – Select available languages.
You want to display Q15 only if the answer to Q14 is YES.
This is how you would set up the columns in the Constraints tab:
Section = Documentation
Question = Do you provide a manual?
Operator = Is equal to
Value = Yes
Result = Select available languages.
Constraint type = is visible
