About this file
This file is a Short Term Contractors Template CBP Contractor Information Sheet, serving as an exhibit to a Statement of Work for visitor access management at U.S. Customs and Border Protection facilities. The template is used to collect and submit detailed information about contract company personnel and visitors requiring temporary access to CBP locations, including contractor company details, on-site points of contact, visitor escort information, and comprehensive personal data for each visitor (names, dates of birth, citizenship status, passport information, visa details, driver's license information, and social security numbers).
The form establishes specific security and access protocols requiring mandatory submission to the Office of Professional Responsibility 72 hours prior to visitor arrival, with submissions directed to SMDSECURITYSERVICESBRANCH@cbp.dhs.gov or faxed to 202-344-1860. The template mandates that all visitors present U.S. Government-issued photo identification, check in and out at security desks, and maintain constant escort by designated CBP employees, with escort ratios specified at one escort per four visitors (1-4 ratio) or 1-1 ratios depending on vetting status. The form accommodates up to 84 visitor entries and incorporates Privacy Act Notice requirements under Public Law 93-579, stating that failure to provide required information may result in access denial.
