About this file
This is a Short Term Contractors Template form used by U.S. Customs and Border Protection (CBP) to process and vet contractor visits to CBP facilities. The form serves as a Contractor Information Sheet that collects detailed personal and identification information for all visiting contractors and their accompanying personnel. Required information includes contractor company details, on-site and CBP points of contact, visit dates and times, purpose of visit, and comprehensive personal data for each visitor including name, date of birth, citizenship status, place of birth, occupation, address, visa information, passport details, Social Security number, and driver's license information. The form accommodates up to 84 visitors per submission.
Submission of this form is mandatory and must be completed 72 hours prior to visitor arrival to the Office of Professional Responsibility via email to SMDSECURITYSERVICESBRANCH@cbp.dhs.gov or fax to 202-344-1860. All visitors are required to present a U.S. Government-issued photo identification upon arrival and must check in and out at the Security Desk. CBP employees designated as visitor escorts are responsible for constant escort of all contractors, with a ratio of one escort per four visitors required, though a one-to-one ratio may be mandated depending on vetting status. Any visitors found unescorted in CBP space will be immediately reported to security and removed. The form operates under the Privacy Act of 1974 and is used to conduct minimum security checks prior to granting limited authorized access to CBP facilities.
