About this file

This file is a Short Term Contractors Template CBP Contractor Information Sheet, a security clearance and visitor access form used by U.S. Customs and Border Protection for managing contractor and visitor access to CBP facilities.

The form captures comprehensive information about visiting contractors and their personnel, including contractor company details, CBP location information, visitor escort assignments, and personal identification data for up to 80 visitors. Required information includes full names, dates of birth, U.S. citizenship status, place of birth, occupation, address, and identification details such as driver's license numbers, passport information, visa numbers, and Social Security numbers. The form mandates that all visitor access requests be submitted to the Office of Professional Responsibility at least 72 hours prior to arrival via email to SMDSECURITYSERVICESBRANCH@cbp.dhs.gov or by fax to 202-344-1860. All visitors must present a U.S. Government-issued photo identification and check in at the Security Desk upon arrival and departure. The form requires visitor escorts to maintain constant supervision, with a minimum ratio of one escort per four visitors, though higher ratios may be required depending on vetting status. Any unescorted visitors found in CBP facilities will be immediately reported to security and removed.
