About this file

This document is an Attachment Price Increase Percentages form associated with Solicitation No. BPM006653, specifically related to the Arizona Department of Transportation (ADOT) and a potential contract renewal process. The document appears to be a standard template for vendors to specify maximum price increase percentages during contract renewal periods.

The attachment outlines a structured approach for potential price increases across four renewal periods, with vendors required to specify their maximum percentage increases (in this instance, all periods are set at 0%). The document emphasizes that increases are not automatically granted and that vendors must provide documentation justifying any requested price adjustments. Each renewal period's potential increase is calculated against the original contract price, and if no percentage is specified, prices will remain unchanged from the original contract term. The form is designed to provide transparency and control in the contract renewal process, with the state reserving the right to review and approve any proposed price changes.
