In order for quote to be considered, the attached Vendor Representation & Compliance with Section 889 updated OCT 2020 MUST be signed and returned with quote.
If there are any Graphic Design Fees, they must be included in the quote.
Samples of art/security plates must be included with quote. Sample must show in detail how it meets the following requirements.
NOTE: The actual STOP ID Numbers below the barcode will be USY00000, where the zeros represent sequential digits ranging from 00001 to 99999. Actual Plate Dimensions are: 2.5” Wide x 2.125” H x .012” Thick. Plate must be aluminum and permanently bonded.

Height - 2.125 inches (54.0 mm)
Width - 2.50 inches (63.5 mm)
Thickness - 0.012 inches (0.30 mm)
Bottom of plate must have ID numbers that are in sequential order
Access to a Secure Online Registration Database to register and keep track of tagged devices.
