About this file

This document is Contractor Prequalification Instructions for Louisville Water Company, a Kentucky water utility. The instructions establish a prequalification system for contractors seeking to bid on Louisville Water Company construction and service projects. Contractors may apply for prequalification at any time through the company's electronic portal at https://louisvillewater.bonfirehub.com/portal/. Prequalification is required for most construction bids and certain service bids or RFP requests, and contractors must achieve prequalified status by the bid submittal date specified in individual bid documents. The application process involves submission through a designated online portal with no guaranteed turnaround time for review completion.

The prequalification categories are organized into six primary sections: Pipeline Contracting (11 categories covering water main installation and repair work ranging from 1" to 60" in various pipe materials including Ductile Iron, PVC, HDPE, Steel, and Pre-stressed Concrete); General Contracting; Service Contracting; Building Services; Environmental Services; and Water Treatment/Pumpage/Storage. Contractors must submit a completed Contractor Prequalification Application (Q-37AV), Employee Personnel Form (A1), Current & Completed Projects Form (A2), Equipment List (A3), two reference sheets per category (A4), a notarized Prequalification Signature Page (A5), current financial statements including three years of income/revenue statements, applicable certifications, affirmative action plans if applicable, articles of incorporation, and a W-9 form. The company recommends limiting applications to three categories maximum to expedite processing. Incomplete applications or failure to respond to information requests will result in application denial. Contractors already prequalified may submit Category Upgrade Applications or Financial Upgrade applications through the same portal.
