a. This is a regular Preventive Maintenance Service Contract.

b. We currently do not have a maintenance contract.

2. How to fill block 17 in the SF1449?

a. 17a. Company Name and Address

b. 17b. Just fill it if the remittance address is different than de company address in

17a.

3. What types of elevators are they? (Traction, hydraulic, machine-room-less (MRL), freight, panoramic, etc.)

a. Traction with a machine room.

4. What are the load capacities and numbers of stops?

a. 1800 kg (4000 pounds), 5 stops in line.

5. What is the brand, model, and age of the elevators?

a. Brand: Otis, Model: GEN2, Age: 20 years.

6. Do the elevators have a machine room, or are they MRL (machine-room-less)?

a. They have machine room.

7. How frequently are the elevators used? (Approximate number of trips per day)

a. There are no daily usage measurements available.
