About this file

This document contains a past performance survey for evaluation of contractor performance on a federal contract. It requests evaluation of technical services, quality of services, timeliness of performance, price/cost control, business relations, and customer satisfaction on a contract between a federal agency and an unnamed contractor providing professional services. Evaluation criteria include exceptional, acceptable, marginal, and unacceptable ratings. The survey collects contract details including period of performance, value, description, and points of contact. Respondents are assured confidentiality, and the completed survey should be returned to the specified Department of State office for consideration in a competitive procurement for support services in areas like interpreting, translating, intelligence analysis, and vetting at high-threat locations globally including an initial focus on the Middle East region. Language requirements include Arabic, Kurdish, Dari, Pashto, and Nepali.
