RFQ No. 15F06718Q0000125 Research Support Services – HIG Attachment 6 Performance Questionnaire

U.S. Department of Justice Federal Bureau of Investigation Professional Services Acquisition Unit Washington D.C

August 20, 2018

RE: Request for Quote (RFQ) No. 15F06718Q0000125 Research Support Services

The Department of Justice (DoJ), Federal Bureau of Investigation (FBI), Professional Services Acquisition Unit (PSAU) is in the process of selecting contractor(s) for award of a commercial Research Support Services contract for the High-Value Detainee Interrogation Group.

One of the considerations in quote evaluation is the verification of the offerors' past and present performance on contracts, which reflect the offeror's ability to perform on the proposed effort. We depend on information received from agencies such as yours, which have had first-hand experience with an offeror, for the evaluation of the offeror's performance on those contracts.

To assist you in preparing your response and expediting your reply, the questionnaire may be filled out by hand and e-mailed directly to me at GLCROSBY@fbi.gov and John Loper, Contract Specialist, Contractor via email at JWLOPER@fbi.gov.

Please call Garland at (202) 324-9013 prior to transmission or if you have any questions. Your completed questionnaire will become a part of the official Source Selection records.

Your help is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
