SECTION C – DESCRIPTION/SPECIFICATIONS/WORK STATEMENT

Macroeconomic Consulting Contract

Background and Need

CMS intends to award a 12 month consulting contract to provide the agency with both short-run

(10 year) and long-range (75 year) macroeconomic projections and other macroeconomic modeling assignments. The projections will be used in production of short-run projections of

National Health Expenditures (NHE) and production of the annual report of the Medicare

Trustees. CMS expects four 12-month option periods to be included to cover recurring annual projection assignments. In each of those option years will be able to choose to proceed with a base package of consulting services or the base package plus an additional expanded package of services.

The prospective vendor must have access to a detailed structural/sectoral model of the U.S.

economy equipped to make 75-year economic projections and a demonstrated ability to adapt that model to the needs of the agency in short-turnaround times. A standard scenario will involve projecting the economy-wide implications of an assumed path of growth for the U.S.

health sector. Multifactor productivity projections plausibly aligned with historic BLS series will also be required. Another important requirement is multidimensional consistency of outputs with historical aggregates.

Purpose

The purpose of this contract is to purchase the Long Term Inter-Industry Tool and to obtain macroeconomic support for long and short term forecasts.

Specific Tasks

(1) Provide CMS/OACT with LIFT 2090 subscription.

(2) Support for NHE 10-year projections.

(3) Support for Data Enhancements.

Implement enhancements to the data underlying the LIFT model for the benefit of CMS.

CMS will contribute resources to the conversion of the LIFT model from a SIC to a

NAICS basis.

(4) Standard Scenario

Provide support for standard long-range macroeconomic scenario calibrated to replicate a state of the world in which SSA economic and demographic assumptions and CMS assumptions about the path of health sector expenditure/aggregate demand all hold.

Although this requirement is needed for TR 2016, it is no longer certain to be a recurring annual need and therefore is not included in the base consulting option package specified for future fiscal years.

(5) Additional 75-year medical spending projections.

The vendor will provide ongoing support to CMS in developing 75 year projections of the health sector and the economy. This support will include: (a) 75-year simulations of the health sector and the economy under alternative assumptions (including assumptions generated by the OACT computable general equilibrium (CGE) model); (b) analysis of the economic implications of different financing options of government medical expenditures (e.g., increasing government deficits, increasing taxes); (c) analysis of

Input-Output data from the Bureau of Economic Analysis.. Long-range projections in coming years will continue to seek to improve modeling the long-range effects of health reform legislation that continues to be implemented. This is a requirement for the current fiscal year, but is only a contingent requirement in future years and for those future years is accordingly included in the expanded option package and not in the base option package for those years.

(6) Special Research Projects

Subsequent to award of the contract the vendor in consultation with CMS shall develop a plan in conjunction with CMS for an in-depth research project aimed at improving and deepening understanding of what the LIFT model can reveal about the U.S. health sector. In the past year the vendor completed a decomposition national health expenditures into the detailed components of supply, the results of which were published in Survey of Current

Business. The supply decomposition was a historical study with a retrospective focus. One possibility for building upon this research might be to shift to a prospective focus aimed at better understanding the trajectory of those same supply elements as part of NHE into the near future. Other possible topics continue to include:

a) Developing a broader macroeconomic and industrial context for the annual

Trustees’ projections.

b) Developing alternative projections given different rates of growth for productivity and the labor force.

c) Evaluating the fiscal and economic sustainability of the long-term health care projections.

d) Investigating the impacts of technology, such as electronic health care records, on the demand, supply, and sustainability of health care expenditures, using microeconomic studies to quantify the industry-level impacts.

e) Assessing the industry and macroeconomic effects of alternative health policies, such as different cost-sharing schemes or the elimination of inefficient spending, that have been quantified at the product or industry level by various microeconomic studies.

f) Developing new data estimates for the differential levels of employer-sponsored health benefits offered across industries. (This effort would require the mining of other data sets, especially non-government data.)

g) Expanding the NIPA-NHE correspondence to include a funding source dimension to the crosswalk. This dimension will help us to trace how funding source might affect the choice of expenditure and vice-versa.

h) Other issues pertinent to long-range projection of health expenditures and their implications as agreed upon by CMS and the contractor.

i) Investigating the long-range implications/feasibility of differential cost growth rates in subparts of the U.S. health sector, such as Medicare, Medicaid and private insurance, particularly in light of health reform legislation.

The listing of topics in this section is meant to be illustrative only of possible additional subjects of in-depth research. Funding for special in-depth research is envisioned for FY 2015, but will be contingent on CMS acceptance of the expanded option package in future fiscal years.

This restriction does not limit the contractor's right to use data or information obtained from a non-restrictive source. Any questions concerning "privileged information" shall be referred to the Contracting Officer.

b. Some data or information may require special consideration with regard to the timing of its disclosure so that preliminary findings which could create erroneous conclusions are not stimulated. Also, some data or information, which relate to policy matters under consideration by the Government, may also require special consideration with regard to the timing of its disclosure so that the open and vigorous debate, within the government, of possible policy options is not damaged.

c. Any questions about use or release of the data or information or handling of material under this contract shall be referred to the Contracting Officer who must render a written determination. The Contracting Officer's determinations will reflect the results of internal coordination with appropriate program and legal officials.

d. Written advance notice of at least forty-five (45) days shall be provided to the

Contracting Officer of the Contractor's desire to release findings of studies or research or data or information described above. If the Contractor disagrees with the Contracting Officer's determination, and if this disagreement cannot be settled by the Contractor and the Contracting Officer in a mutually satisfactory manner, then the issue will be settled pursuant to the "Disputes" clause.

e. Any presentation of any report, statistical or analytical material based on information obtained from this contract shall be subject to review by the COR before dissemination, publication, or distribution. Presentation includes, but is not limited to, papers, articles, professional publications, speeches, testimony or interviews with public print or broadcast media. This does not apply to information that would be available under the Federal Freedom of Information Act.

f. The COR review shall cover accuracy, content, manner of presentation of the information, and also the protection of the privacy of individuals. If the review finds that the Privacy Act is or may be violated, the release/use of the presentation shall be denied until the offending material is removed or until the Contracting Officer makes a formal determination, in writing, that the privacy of individuals is not being violated.

g. If the review shows that the accuracy, content, or manner of presentation is not correct or is inappropriate in the light of the purpose of the project, the COR shall immediately inform the Contractor, in writing, of the nature of the problem.
