Replace potable water line automatic flush valves at Katherine Landing AZ, Callville Bay NV and Echo Bay NV Water Distribution Systems

STATEMENT OF WORK

Scope of Work

SCOPE OF WORK

Project Identification: The purpose of this contract is for a contractor, with the appropriate credentials and Licensure to Install up to fifteen (15) water main automatic flushing valves. The contractor will install up to five at the following locations Katherine Landing and Echo Bay. The Contractor will install up to Three at Callville Bay and up to two at Las Vegas Bay. The Contractor will install new piping, flanges, valves, taps and all associated piping where necessary. New piping must be at least 2’ deep and existing water mains are up to four feet deep. Shutoff valves are required at each flusher to facilitate future servicing of units. Exact placement of flush valves will be identified at the pre-construction meetings. The new auto-flushers and plumbing must be installed one at a time and tested for proper operation to minimize any chance of down time for downstream customers. Installation of flushers will be scheduled with and observed by designated COR. New flushers will be compatible with current Mueller phone app controller system. New auto flushers must be NSF 61 compliant for use with drinking water. Flush valves must be comparable and compatible to government furnished equipment. Hot tapping to facilitate installation is preferred method as to not affect water service in the areas. Water Outages: If the instillation requires a water outage to a concession facility or a residential area, the Contractor shall notify the COR 48 hours in advance. The Contractor shall provide the COR, the length of the proposed outage. Records must be maintained and conveyed to the NPS to include Serial number, size, Make and Model, Date installed, GPS location for each device, user manual and must be conveyed in a digital format such as excel and/or PDF.

PERFORMANCE PERIOD:

This contract will have an initial period of performance of 90 calendar days from time of award.

CONTRACTOR PERSONNEL

The services will be performed by a quoter with the appropriate credentials and licensure.

QUALITY CONTROL - All work shall be performed in accordance with the following:

Industry Standards:

The Uniform Plumbing Code; Recommended Practice for Backflow Prevention and Cross- Connection Control.

The Manual of Cross-Connection Control.

State Administrative Codes:

Arizona Administrative Code (AAC): title 18 - environmental quality, chapter 4 - department of environmental quality safe drinking water, supp- 04-1, article 1 - general requirements, Section R18-4-115 - Backflow Prevention.

Nevada Administrative Code (NAC) and Nevada Revised Statutes (NRS): NAC 445A.67245

Water Outages: If the instillation requires a water outage to a concession facility or a residential area, the Contractor shall notify the COR 72 hours in advance. The Contractor shall provide the COR with the BPA number, and the length of the proposed outage.

All items must be NSF60 approved

PHYSICAL SECURITY

The site is not secured and is accessible at the discretion of the selected quoter, operating under standard hours of operation

HOURS OF OPERATION/WORK SCHEDULE

Standard hours of operation/work shall be Monday through Friday between the hours of 7:00 AM and 4:00 PM unless alternate hours are approved by the Contracting Officers Representative (COR). The Contractors proposed quote price should be based on working only during standard workdays and hours.

Work Schedule: As soon as the Contractor receives the "Notice to Proceed" letter from the Contracting Officer he/she shall develop a work schedule and submit it to the COR for approval, starting with the orientation meeting. The COR will use this schedule to provide to coordinate with other involved NPS work groups as well as concessionaires. The schedule should consist of the targeted day of the week for weekly service during the high use season March-October, as well as the date of first service for the year.

CONTRACTOR USE OF SITE

General: Limit use of the site during the construction period to the project area limits as outlined on the attached site maps. Do not disturb portions of the site beyond the project area limits.

Park Entry: The contractor shall furnish a list of personnel with phone contact information.

Vehicles associated with the contractor shall be waived entrance fee upon entering in a marked company vehicle or by providing the names of employees entering in personal vehicles. The entrance fee is waived only for the purpose of reporting directly to a specific work location. Any deviation for recreational purposes is subject to payment of required entrance fees.

Site Access: The contractor shall access the project site as outlined on the attached site maps. Do not drive vehicles off road or access non-project areas without permission from the contracting officer or park superintendent.

Park Regulations: Ensure all personnel (contractors, subcontractors, and vendors) entering or working in the park are familiar with and obey all park regulations. Law enforcement rangers will intervene if they perceive any unsafe or unlawful activity.

Working Hours: Regular work hours shall be limited to daylight hours from 7:00 a.m. until 5:00 p.m. Monday through Friday, excluding government holidays. Work outside regular hours requires approval from the contracting officer.

No Smoking Policy: Smoking is prohibited inside or outside within 25 feet of any buildings in the park. This applies to existing buildings, buildings under construction, and buildings under renovation. Discarding of tobacco products or debris anywhere other than into designated tobacco receptacles is considered littering and subject to fines.

Staging and Storage: Confine storage of equipment and materials to the staging / lay-down / storage areas identified on the attached site maps or as established at the preconstruction meeting.

Parking: The contractor shall park vehicles and heavy equipment in designated areas for contractor use or as established at the preconstruction meeting. Vehicles are not permitted within long-term or overnight parking along roads or in associated overlooks / pull-outs.

Driveways and Entrances: The contractor shall keep driveways and entrances serving the premises clear and available to the government, government employees, and emergency vehicles at all times.

Preservation of Natural Features: Prevent damage to natural surroundings. Restored damaged areas and replace damaged trees, vegetation, and grass at no expense to the government.

CONSERVATION OF UTILITIES

All Contractor personnel shall make every effort to conserve energy when performing

One Prebid site visit will be scheduled prior to close of solicitation for all interested bidders to attend

After contract award the Contractor shall attend an orientation meeting to be conducted at the Lake Mead Maintenance Warehouse Building, located at 101 Katzenbach Dr, Boulder City, NV 89005.

Lake Mead personnel will be present to go over park regulations and the specific requirements of the project with the contractor.

TECHNICAL REQUIREMENTS

Contractor is required to submit at minimum a daily construction progress report, plan for the next day, before and after pics that assist in documenting the installation process and progress.

Work includes associated mechanical joints needed, parts, fittings, thrust blocking, delineation, flagging, traffic control, trenching, grading, backfill and labor to provide a functioning water distribution system that satisfies the needs of the NPS unit as determined by NPS personnel.

The design of this project must be approved by NPS LAKE Utility Office.

The contractor will be responsible for any required permits or compliance needs from the State of Arizona and Nevada. All new utility work will comply with AWWA, NPS and State of Arizona and Nevada regulations. Permits must be stamped by a AZ, NV P.E. when required. Copies of all permit submittals and approvals will be provided to NPS asap.

GENERAL DEFINITIONS

QUALITY ASSURANCE. Actions taken by the Government to assure services meet contract requirements.

QUALITY CONTROL. Those actions taken by a Contractor to control the performance of services to ensure services meet the requirements of the SOW.

TECHNICAL DEFINITIONS

Technical Definitions are described in the BPA manufacturers Operating and Service Manual, and Section 3 of this SOW

GENERAL INFORMATION

The Contractor shall furnish all labor, materials, permits, disposal fees and equipment necessary to complete the project scope of work.

CERTIFICATES/CREDENTIALS

Certifications/Credentials at a minimum should include all pertinent state and federal certifications held by the contracting company and individual technicians who will perform the work onsite.

Capability/Capacity

Capability/Capacity at a minimum should describe the number and credentials of all credentialed employees involved in the all aspects of the project.

GENERAL INFORMATION

The contractor shall have access to the publications listed below to ensure the work is accomplished in accordance with the manufacturer's recommendations and that the work is properly documented prior to submittal to the Government.

PERFORMANCE REQUIREMENTS SUMMARY

The final finished product shall be validation/certification that all BPAs listed on the Price Schedule are fully functioning and have passed all required tests. If a BPA does not pass the initial test, repairs or adjustments shall be made until the BPA can successfully pass the test.

PERFORMANCE EVALUATION

The Performance Evaluation will be based on the spot inspection performed by the COR and the reports submitted by the Contractor.

CONTRACTOR PAYMENT

Payment will take place after the Contractor provides the Government with all the documents listed.

REQUIRED REPORTS

GENERAL INFORMATION

All reports shall be submitted in electronic format, plus one signed original paper copy set.
