The purpose of this amendment is to extend the response due date/time and clarify delivery terms and conditions.

The contractor is responsible for ensuring direct delivery/shipment of furniture to the helipad located at Hilltop, Arizona (directions to helipad included below). Contractor must notify school point of contact as to exact day shipment is expected to be delivered to helipad. Point of contact at school is Juanita Benally. She can be reached by telephone at

(928) 550-3389.

In addition, contractor personnel travel costs (ground transportation to helipad and helicopter transportation in/out of canyon) must be included in a separate line item in

140A2323Q0444/0003 quote. Two people are recommended to unpack, assemble and set up the new furniture on the day after the delivery of shipment to helipad.

Overnight stay is not needed. Also, packaging from the new furniture must be removed by the contractor to a designated location in the canyon.

If some items are out-of-stock at the time of the initial delivery, contractor must include additional personnel travel costs (ground transportation to helipad and helicopter transportation in/out of canyon) on a separate line item if further unpacking, assembly and set up is needed for any items shipped late.

Overnight stay is not needed. Also, any additional packaging from the new furniture must be removed by the contractor to a designated location in the canyon.

The cost of slinging the furniture into the canyon is not the contractor's financial responsibility and should not be included in the quote. Contractor is also not required to arrange for costs of helicopter slinging out packaging from canyon.

Delivery point to helipad is located in a remote area. There are no services for several miles and contractor must refuel prior to traveling to and from delivery point.
