The GTM will also perform random inspections to verify that work is being done in accordance with the terms of the contract.

G.7 STANDARD

The Contractor shall perform all work required in a satisfactory manner in accordance with the appropriate SOW paragraph and Service Delivery Summary. No more than three deficiencies per month will be allowed for the daily cleaning services (restrooms and office cleaning), no more than two deficiencies per month will be allowed for the weekly services, and no more than 2 deficiencies per period will be allowed for quarterly and semiannual services. The GTM shall notify the Contracting Officer for appropriate action if the maximum number of deficiencies is exceeded.

G.8 PROCEDURES

The GTM will inspect all work to ensure Contractor compliance with the appropriate paragraphs of the Statement of Work (SOW) each time the service(s) is performed and record results of the inspection, noting the date and time of inspection. If an inspection indicates unacceptable performance, the GTM will notify the Contractor of the deficiencies for correction using the CLB. Other means such as e-mail may also be used to notify the Contractor of deficiencies.

Any customer/employee that observes unacceptable services, either incomplete or not performed, for any of the above performance objectives should notify the GTM, who will in turn record the complaint in the Communication Log Book (CLB). The GTM will consider the customer/employee complaint valid upon receipt from the customer/employee. The GTM should inform the customer/employee of the approximate time the unacceptable performance will be corrected and advice the customer/employee to contact the GTM if not corrected. The GTM will consider customer/employee complaints as resolved unless notified otherwise by the customer/employee.

The GTM shall keep the CLB in a location where the Contractor’s Quality Control Inspector (QCI) or janitor shall view the log for customer/employee complaints on a daily basis. For the daily and weekly services, the Contractor will be given one scheduled work day after the date noted in the CLB to correct any deficiencies. For the periodic services (quarterly and semiannual), the Contractor will be given one scheduled work week to correct any deficiencies.

The QCI or janitor shall note remedy in the CLB next to the complaint.

If the Contractor disagrees with the complaint after investigation of the site and challenges the validity of the complaint, it shall notify the GTM. The GTM will conduct an investigation to determine the validity of the complaint. If the GTM determines the complaint as invalid, he will document the findings and notify the customer/employee. The GTM will retain the annotated copy of the written complaint for his files. If, after investigation, the GTM determines the complaint as valid, the GTM will inform the Contractor and the Contractor will be given an additional day (in the case of daily and weekly services) to correct the deficiency. A deficiency will not be recorded if proper and timely correction of the unacceptable condition(s) is accomplished.

The janitor shall log remedy to the customer/employee complaint with actions taken in the CLB.

These will be kept on file for monitoring performance. Recurring customer/employee complaints are not permitted for any of the above service items. If a repeat customer/employee complaint is received indicating a recurrent deficiency, the GTM should contact the Contracting Officer for appropriate action.

G.9 MEASUREMENT AND PAYMENT

Payment will be made on a monthly basis at the contracted unit prices based on services performed during the previous month. The Government will not pay for work that has not been performed in accordance with the terms of this contract shall. The Contractor shall not include in its monthly invoices any work that has not been completed. Payment will only be made for completed work.

G.10 FACILITY ACCESS AND BACKGROUND INVESTIGATIONS OF CONTRACTOR

EMPLOYEES

Contractor Background Investigation

The Contractor shall conduct its own, background check of any employee requiring unescorted access to the facility and submit the results to the contracting officer prior to the start of work.

The Contractor is responsible for the cost of this background investigation. Background investigation will be collected after Contract Award and only the contractor awarded the contract will need to submit one.

A Washington State Patrol WATCH background check is one acceptable source. For more information, visit http://www.wsp.wa.gov/crime/chrequests.htm.

Building Access

Access to a Government facility shall be limited to only those individuals employed by the Contractor. The Contractor shall not admit others to the building. Any Contractor employee who accesses the building outside of business hours is responsible for ensuring that the building is secure (i.e., doors locked and security system armed) when he or she leaves. The Government reserves the right to remove any Contractor or subcontractor employee that is, in the Government’s estimation, a security risk to the Government.

Note: Subcontractor employees performing non-routine work will not undergo a background investigation and thus will not be granted unescorted access to the building. Should the Contractor hire a subcontractor for any portion of the work, it must coordinate performance of the work with the Government Technical Monitor in order for a government employee to be present during performance. The Contractor shall allow enough time to coordinate such work with the GTM.
