About this file
This document is an Affirmation and Disclosure Form related to a state contract, designed to ensure compliance with executive orders regarding offshore services and data location. The form requires contractors to disclose the principal location of their business, the locations where services will be performed, and where state data will be stored, both for the primary contractor and any potential subcontractors. Contractors must explicitly affirm that they understand and will comply with prohibitions on performing offshore services, locating state data offshore, or purchasing from Russian institutions or companies.
The form stipulates that contractors have a continuous duty to disclose any changes in service or data location to the state, with the understanding that the state reserves the right to immediately terminate the contract if services are moved outside the United States without a signed waiver. By executing this form, the contractor acknowledges that it is part of any potential contract with the state and agrees to immediate notification of any location shifts before, during, or after contract execution. The document is structured to provide transparency about service and data locations while maintaining strict compliance with state regulations regarding offshore operations.
