About this file
This document is an Affirmation and Disclosure Form, revised in March 2022, designed to ensure compliance with Executive Orders regarding offshore services, data location, and purchasing restrictions. The form requires contractors to disclose the principal business location, service performance locations, and data storage locations for both the contractor and any potential subcontractors.
The form mandates that contractors affirm they understand and will comply with prohibitions on offshore services, locating state data offshore, and purchasing from Russian institutions or companies. Contractors must immediately notify the state of any changes in service or data location, with the understanding that the state reserves the right to terminate the contract if services are performed outside the United States without a signed waiver. The document serves as a critical component of contract agreements, requiring full transparency about operational locations and potential international service delivery.
