About this file

This document is an Affirmation and Disclosure Form (Revision 3/22) designed to ensure compliance with Executive Orders regarding offshore services and data location. The form requires contractors to disclose the principal business location, service performance locations, and data storage locations for both the primary contractor and any potential subcontractors. The form specifically emphasizes prohibitions on performing services offshore, storing state data outside approved locations, and purchasing from Russian institutions or companies.

The form mandates that contractors provide complete transparency about their operational locations and agree to immediately notify the State of any changes in service or data location. Contractors must understand that the State retains the right to terminate the contract if services are moved outside the United States without a formal signed waiver. The document serves as a critical compliance mechanism, requiring the contractor's authorized representative to acknowledge and sign, confirming their understanding that this disclosure is an integral part of any potential contract with the State.
