About this file
This is a Vendor Accessibility Disclosure Form (Appendix C) issued by the Colorado Community College System (CCCS) as part of RFP 27-001 for CLR LER System Implementation. The form requires vendors submitting proposals to disclose information about their products or applications and their compliance with Web Content Accessibility Guidelines (WCAG) 2.1 Level AA standards. Vendors must provide their company information, identify up to two products being proposed, and certify the accessibility status of those products. The form allows vendors to indicate whether their products are fully WCAG 2.1 Level AA compliant, compliant but working toward compliance, or non-compliant with no plans for compliance. Vendors selecting either of the first two options must submit a completed VPAT 2.5 WCAG document alongside this disclosure form.
The accessibility disclosure requirement reflects CCCS's commitment to facilitating access to services, programs, and activities for the broadest possible audience in accordance with the Governor's Office of Information Technology Rules Establishing Technology Accessibility Standards and Board Policy 3-125. The form applies to all information and communication technology developed or maintained by CCCS entities, including websites, instructional materials, online services, software applications, multimedia content, and kiosks. Vendors must certify under penalty that the information provided is true and correct, understanding that false statements may result in disqualification from the procurement process.
