Virginia's Department of Forensic Science has awarded contract PO05420498 to Proforma Graphic Information Services for the procurement of printed forms. The contract, awarded on July 27, 2026, has a cumulative value of $4,799.00. The primary deliverable consists of Certificate of Blood Analysis (Breath) forms, 8 by 11 inches in dimension, with perforated tear-and-burn 2-fold configuration and tractor strips positioned on the sides (Form 250-F300, Revised version). The order encompasses 20,000 individual units at a unit price of $0.24 per form, with delivery directed to Debra Myers.

This procurement supports the Department of Forensic Science's operational requirements for breath alcohol testing documentation and analysis reporting. As a printed forms supply contract, it falls within standard government commodities procurement and enables the department to maintain necessary evidentiary documentation procedures for forensic testing operations. The contract structure and unit pricing indicate a straightforward goods acquisition with no identified period of performance details beyond the award date.
