Jotform Enterprise Base Package (includes 5 Permitted Users)

The University of Nebraska has awarded a software license renewal contract to JotForm Inc. under contract number CW49631, effective September 17, 2026, through September 17, 2027. The contract value is $4,200.00 annually, representing the cost after application of a 30% nonprofit discount to the base rate of $6,000.00 per year. The deliverable consists of the JotForm Enterprise Base Package, which includes five permitted users and provides web-based form management and distribution capabilities supporting institutional workflows, including the Risk Classification Self Assessment tool utilized by the University's Information Technology Services Security and Systems teams.

This contract is a renewal addendum (Addendum #6) to an original Software License Agreement executed on September 17, 2020, with a cumulative value of $27,257.30 across all amendments through 2027. The procurement was conducted on a non-competitive basis under University of Nebraska Policy RP-6.2.1.5.b, with justification based on the existing dependency of university workflows on JotForm's functionality and the substantial developer involvement that would be required to migrate the current forms to alternative platforms. Additional permitted users beyond the contracted five are available at $948.00 per user annually, invoiceable during the contract term without formal amendment.
