The Florida Department of Health issued a blanket purchase order contract (C6BD5A) valued at $7,000.00 for swimming lesson vouchers under the provisions of Florida Statute 514.073. The contract covers swimming instruction for eligible children aged four years and younger, with lessons provided at $25.00 per session. Each participating child is authorized to receive a maximum of eight lessons, with payment triggered upon completion of instruction. The coverage period extends from September 1, 2025 through June 30, 2026, and the contractor is required to submit signed attendance sheets with each invoice.

The contract was awarded on January 26, 2026, with a completion date of April 29, 2026. Invoices are submitted upon child completion of lessons rather than on a time-and-materials basis. The prime contractor must maintain documentation in the form of completed sign-in sheets for all lessons provided to ensure compliance with program requirements and to support billing verification.
