This $1.5 million project grant from the National Institute on Aging, part of the Department of Health and Human Services, will support Memorywell, Inc. in delivering services under the Aging Research federal grant program (CFDA 93.866). Specifically, Memorywell will partner with the University of California San Francisco Acute Care for Elders unit to adapt its life story solution for use in acute care hospital settings, where brief patient life stories would be incorporated into electronic health records. Over the one-year period from September 2021 to August 2022, Memorywell will conduct user research and co-design a system to capture and integrate life stories of patients with Alzheimer's or other dementias directly in hospital EHRs. It will then create 25 patient life stories for implementation and evaluation at the UCSF unit to establish the feasibility and acceptability of incorporating life stories in acute care. This project aims to improve person-centered care for dementia patients in hospitals through use of life stories in EHRs.
