The Department of Health and Human Services Agency for Healthcare Research and Quality awarded the University of Utah $248,956 on April 1, 2026, under the Research on Healthcare Costs, Quality and Outcomes program (CFDA 93.226) to develop and test health information technology tools supporting an age-friendly learning healthcare system for outpatient geriatrics care.
The funded work delivers a point-of-care clinical decision support system that integrates patient-generated health data and electronic health record data to support age-friendly health system care across the "4Ms" framework: what matters most, medication, mentation, and mobility. The project also includes population-level reporting tools to help institutional decision-makers assess and refine age-friendly care practices, including falls assessments, medication optimization, and dementia and depression screening aligned with individual patient goals and values.
