The recipient will conduct a randomized trial of CV Prevention Choice, a next-generation decision aid that incorporates multiple cardiovascular risk-reducing strategies beyond statins. The intervention builds on preliminary evidence from an observational study showing that patients exposed to Statin Choice, an earlier EHR-based decision aid deployed throughout Cleveland Clinic Health System in 2017, demonstrated a 32 percent higher probability of filling statin prescriptions and sustained adherence at one year. The trial will also evaluate strategies to increase physician utilization of the decision aid, as prior adoption reached only half of primary care physicians despite high acceptability. The work addresses a significant gap in cardiovascular disease prevention: nearly half of U.S. adults could benefit from statins, yet many at highest risk are never offered them, and approximately half of those who initiate therapy discontinue within one year.
