The project will conduct two parallel randomized trials comparing the effectiveness of two weight-regain prevention approaches—switching from GLP-1 to generic non-GLP-1 oral weight management medications, and gradual GLP-1 dose tapering—against continuation of high-dose GLP-1 as the standard of care. The trials will measure weight regain at six months (primary outcome) and weight trajectory over twelve months (secondary outcome), and will include secondary analyses of body composition changes, metabolic risk markers, quality of life, appetite control, medication adherence, and cost. The project will integrate patient-reported outcomes and qualitative evaluation into the analysis. Work is performed at Massachusetts General Hospital's Research Management Division in Boston, Massachusetts. The period of performance runs from September 1, 2026, through June 30, 2031.
