The research aims to determine whether retinal imaging—using fundus photography, fluorescein angiography, optical coherence tomography (OCT), and optical coherence tomography angiography (OCTA)—can illuminate the underlying mechanisms, pathology, diagnosis, and prognosis of chronic kidney disease. Specific objectives include testing associations of retinal vascular features with kidney expression profiles, kidney histopathology, and CKD progression; determining the ability of comprehensive eye imaging to identify meaningful CKD subgroups including clinicopathologic kidney diagnoses and novel molecular subtypes; and identifying new patterns of multimodal retinal imaging features associated with CKD subtypes using artificial intelligence and machine learning. The retinal imaging suite will be added to an existing cohort that employs standardized kidney biopsies to characterize molecular mechanisms of kidney injury with detailed histopathologic analysis. Performance occurs in Seattle, Washington, with a period of performance from September 1, 2026, through May 31, 2030.
