The award funds a pragmatic stepped-wedge cluster randomized trial testing infection mitigation measures (IMM) in patients with advanced alcohol-associated liver disease (AALD). The trial will enroll approximately 1,000 patients with AALD at four leading U.S. centers and compare IMM against usual care. All sites will begin in a control period, then be randomized at regular intervals to crossover to the IMM intervention. Patients will continue to receive standard-of-care management for AALD throughout. The primary endpoint is the rate of hospital-acquired infections (HAI) during the index hospitalization. Secondary endpoints include rates of secondary and tertiary HAI, infections with multidrug-resistant organisms, sepsis, acute kidney injury, multiorgan failure, mechanical ventilation, length of stay, readmission, and survival.
