The planned project will provide pre-release case management services to 100 incarcerated individuals within at least one county jail. The services aim to eliminate health and social services gaps, including arranging linkages to a medical home, testing for HIV/Hepatitis C, early detection and treatment of TB, management of sexually transmitted infections, and evidence-based case management and peer navigation. The project will leverage Southside Medical Center's existing Ryan White Part A services and PrEP program to keep people living with HIV virally suppressed and prevent new HIV infections. The project will also provide treatment for chronic and acute illnesses for the justice-involved population. This 2-year award will be active from December 1, 2024 to November 30, 2026.
